did you two drs miss the point that the person is in PAIN and can't get appointment with specialist immediately--like almost anyone in similar situation--
what other options does he have except visit to ER or doc-in-the-box--SUFFER???
I don't have any sympathy for doctors who apparently think patients who aren't cured by one physician's response to clinical symptoms should just be quiet...
that attitude apparently is the picture of today's medicine--much less the future...

the patient went for symptoms of an acute situation--he was treated--the treatment was ineffective--
THAT is the problem...

Didn't miss the point that he was in pain...or needed some type of symptomatic relief.

Just me, maybe - I see a doc-in-the-box and an ER as two entirely different things. I've got a great family doc but when I woke up one saturday morning with a horrible ear-ache, I went to a local walk-in clinic and rec'd excellent care: a full exam and prescriptions for both an antibiotic and a pain killer.
My family doc doesn't keep week-end hours and I wouldn't have dreamed of going to an ER.
Isn't that what doc-in-the-boxes are for?

That's fine, he can come back to the ER for a repeat treatment. But this time around - we have to be sure that there's nothing else going on like an epidural abscess, another emergent issue like mets/lytic lesions, etc. But if it's still pain that's bothering him or her (from his myofascial syndrome), he/she should get another round of pain meds, and another script of meds to get him through to make it to his "definitive" treatment of facet shots by his pain doctor/back pain specialist.

I don't need your sympathy if you don't understand how medicine works... and if you could please carefully read my previous posts. I did NOT say he should not come back to the ER - I suggested that if he wants definitive treatment that he should wait for his back specialist. I also SAID that if he comes back to the ER, the same round of treatment will be given with a tweak here or there - but no facet shots from me (I'm not trained with that particular procedure - sorry).

This is how it works - pain needs to be controlled, but definitive treatment is definitive treatment. If he wants to come back to the ER - that's fine. But he has to understand that if he's looking for a procedure that's not commonly practiced in the ER, don't expect that. And I'm not calling in the back doctor from home unless there's another issue that we're not entertaining (the patient knows the doctor personally and the back doctor doesn't mind, etc) here.

If you really have intractable back pain - I have no problem admitting you to the hospital for IV narcotics, but you have to understand that you are liable for all the bills. And this is one reason for our rising healthcare costs in this country. Everyone wants something now now now.

But I understand if it's EXTREMELY painful, come back to the ER. I'll be glad to care for you.

Doc in the box and ER are two different things, loves2.

Usually the 'difference' for most people who are picking the ER for their non-emergency condition is that they know they can get out of paying the bill. The doc in the box charges up front.
