    Observation: Observed broken/buckling service area counter (next to Slushie machine).
    Correction: Repair the counter.

Observed worker place new rice on service line and top with old rice from line, but put new time onto container. Understand that some of this rice has already been held and times are different. Instruct workers about appropriate process. Good handwashing observed. Good reheating practices. Permit issued, post in public view. 400 ppm at 3-c sink.
  • Critical: Time as a Public Health Control* (corrected on site)
    Observation: It appears that time tracking of food is not entirely truthful. A yellow sticker indicated that the corn salsa would expire at 6 PM. Chart indicates this product has a 4 hour time. However, at 2:10 PM product had elevated temperature of 49.5 F. Apparently this had been on service line longer than 10 minutes.
    Correction: The system you have has to work as planned. PIC had salsa discarded.
  • Physical Facilities in Good Repair (repeated violation)
    Observation: Air conditioner causing condensate to drip from ceiling vents and from ceiling panels where water has pooled.
    Correction: Repair compromised ceiling panel.
