Notes for repair: Facility needs to be cleaned, Broken tiles under fryers need repair, hose at the mop sink needs a backflow, mop sink is breaking (needs repair), chicken walk-in does not close properly, Pipe coming from walk-in is not over a drain (PVC pipe needs to drip into drain), Prep sink and pipes for the hot water heater both have leaks, trash needs to be taken out regularly. There is also a lot of standing water behind the fryers, outside the chicken walk-in and outside the freezer walk-in.

Observation: Observed employees fail to wash hands after entering the kitchen and after using the phone, before returning to work.
Correction: Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task.
