Notes for repair: Facility needs to be cleaned, Broken tiles under fryers need repair, hose at the mop sink needs a backflow, mop sink is breaking (needs repair), chicken walk-in does not close properly, Pipe coming from walk-in is not over a drain (PVC pipe needs to drip into drain), Prep sink and pipes for the hot water heater both have leaks, trash needs to be taken out regularly. There is also a lot of standing water behind the fryers, outside the chicken walk-in and outside the freezer walk-in.

    Observation: Observed employees fail to wash hands after entering the kitchen and after using the phone, before returning to work.
    Correction: Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task.

    Observation: Observed the top of the mixer/mixer shield soiled and several dishes soiled and with date mark labels still on them.
    Correction: Make sure dishes and equipment are cleaned thoroughly after each use. Also allow dishes to air dry properly.

    Observation: The hood vent is broken and needs repairing on the side closest to the walk-ins and there is a vent missing. The door to the chicken walk-in doesn't close all the way. the bottom seal to the freezer is not working well (creating standing water in front of the door).
    Correction: Have these items repaired properly.

    Observation: Observed the mixer and several dishes that were soiled.

    Observation: Observed employee chewing gum in the kitchen.
    Correction: Get rid of gum. Employees can not chew gum or eat in the kitchen.

    Observation: Observed chicken in the walk-in between 44-46 degrees.
    Correction: Door is not closing all the way, so ambient temperature in the refrigerator is 43 degrees. Have door repaired and make sure temperature of food is held below 41 degrees.

The vent hood and filters in need of cleaning, the floor tiles need to be repaired near the mop sink, the speed racks and food racks in the walk-in refrigerators are in need of cleaning, the back prep sink and 3 compartment sink faucets have leaks, the air vents/ceiling tiles near the friers are in need of cleaning, and the condensate pipe from the walk-in needs to be dripping into a drain and not on the floor.

    Correction: Make sure the dishes are cleaned thoroughly before putting them up. Also make sure to remove date mark labels completely from clean dishes.
