    Observation: Observed the ambient air temperature in the service area reach-in refrigerator that holds the milk registered 45 degrees.
    Correction: Cold foods must be maintained at 41 degrees or below. Adjust the thermostat and have the unit serviced to enable it to maintain proper temperature. Refrain from using milk if held at improper temperature for 4 hours/more.

05/29/2015Routine
This visit is due to a complaint in regards to workers touching ready to eat foods with their bare hands and storing doughnuts on rusty shelving. Upon inspection, workers were using gloves/deli paper to dispense ready to eat foods. I saw no evidence of rusty shelving.
No violation noted during this evaluation.
09/16/2014Complaint
New shelves installed in small storeroom. Shelves in back storeroom have been painted. Daily cleaning is necessary. Keep coffee filters protected. Personal drinks to have lids and straws. Gloves are being worn when handling the finished product. Annual permit issued.

    Observation: No ventilation in one restroom.
    Correction: Provide ventilation.
  • Physical Facilities in Good Repair
    Observation: Wall by 3-c and handsink is not smooth and cleanable.
    Correction: Wall to be resurfaced so it is easily cleanable.
  • Physical Facilities - Cleaning Frequency and Restrictions (repeated violation)
    Observation: Floor in corner behind grease interceptor is dirty. Grease collecting on wall by 3-c sink where strainer is stored.
    Correction: Clean thoroughly.
08/28/2014Follow-up
Above violations have not been corrected. A 30 day extension is allowed to correct the above. Call me when complete. Another extension will not be issued. All surfaces in your establishment are to be smooth and easily cleanable. They are to be kept clean as well. The soiled items marked for correction have been neglected for some time.

    Observation: Shelves in storeroom are no longer smooth and cleanable. Shelves storing flour bags are bare wood.
    Correction: Resurface shelves. Make shelves cleanable, seal.
  • Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils (repeated violation)
    Observation: Hood filters and large mixer splash area found dirty.
    Correction: Clean thoroughly.
  • Refuse - Covering Receptacles (repeated violation)
    Observation: Dumpster or outside refuse container was open or uncovered.
    Correction: Cover all waste containers when not in continuous use.

    Observation: Employees observed handling ready-to-eat (RTE) food with their bare hands.
    Correction: Provide suitable utensils such as deli tissue, spatulas, tongs, single-use gloves, or dispensing equipment to handle RTE food to prevent contamination from hands. Gloves were put on.

    Observation: Observed containers of chocolate frosting placed on floor to fill.

Complaint received regarding a dirty display case, possible mouse droppings, dirty floors and reusing deli papers. Deli papers are considered a single use item. Use once and discard. Used deli papers are placed in a spare box and they are not reused. This is a convenience to operator (less steps). No mouse problem in establishment. Display case appeared clean. Floors were not dirty. Signs of sugar near showcase, dining area was clean.
No violation noted during this evaluation.
