Walk-in cooler has several broken tiles inside, around the walls, that need to be replaced, there is a leak in the ceiling of the walk-in (leaks when raining) that needs to be fixed, and the tiles on the outside of the walk-in (along the wall) have not been sealed yet. The light shields in the kitchen have not been fixed, need to have the tubes on the lights or a solid plastic shield. Continue having the pest control company treat for roaches (a few visible live roaches in the facility, last treatment on 12/9/15). Work on cleaning the kitchen, equipment, and walk-in thoroughly and regularly to help remove pests.

    Observation: Observed employees fail to wash hands between tasks. Hands need to be washed after handling the register, before handling food.
    Correction: Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task.

    Observation: Observed below 135F: top halves of corn on the cob 123F (above water level in pan), green beans 86F (steam table broken). Hold times on both items exceeds 4 hours.
    Correction: Keep water level high enough to cover entire cob of corn to maintain temperature. New steam table on order (discarded green beans and gravy). Maintain hot TCS food at >135F.

All drains are repaired and flowing. There is a formal policy implemented authorizing and ordering all unit managers and persons in charge to cease food service and notify the local health department whenever there is an imminent health hazard. Permit has been reissued as agreed by Health Director.

Management stated that this has been an on going issue for about 2 weeks .
In accordance with 12 VAC 5-421-3870 your permit to operate a restaurant is suspended. The former permit holder has the right for a hearing. The request for a hearing shall be in writing. The written request shall be filed with Henrico Health Department by the former holder of the permit. If written request for a hearing is not filed within 10 working days, the suspension is sustained. The former permit holder of a suspended permit shall be afforded an opportunity for an informal hearing, within three working days of receipt of a request for the hearing. This is by order the Health Director for Henrico Health Department. This facility must be completely disinfected (floors, walls, and ceiling), all open containers of food must be discarded, all open single use items must be discarded and all utensils must be rewashed and sanitized.

    Observation: Observed raw human waste come out of the floor drain when the toilet was flushed. Also, raw sewage was coming out of the floor drains in the kitchen. The rest-room door were locked so that the public could not use the rest-rooms. Manager opened the door to the restroom and flushed the toilet to show me the problem. Sewage immediately came out of the floor drain.
    Correction: Clean out drains and sanitizer permises.

    Observation: Observed the restaurant cooking and serving the public with raw human sewage on the floor from drains backing-up
    Correction: Operation must close. Manager closed after asked by EHS to voluntarily close.

Complaint: 3/11/13 - men's restroom out of order for 3 weeks. I spoke with the manager. Restrooms are closed to the public. There was a problem with the plumbing backing up. It was determined to be a problem in the line belonging to the county. The county has scheduled a subcontractor to repair the issue. The men's restroom is still functional and is being used by employees. Because there is public seating, a restroom must be made available to the customers (upon request).

Investigated fire call - biscuit oven failure and fire. Inspected facility and instructed manager to discard all food in prep, holding, and cook stages of operation. Powder charge discharged in kitchen on all kitchen surfaces. All surfaces are to be washed, rinsed and sanitized and all paper products to be discarded. Call for reinspection by health department prior to opening.
