Observation: Ice making water is coming into contact with mold on the interior surfaces of the ice machine.
Correction: Empty, disassemble, clean, and sanitize the ice machine according to the manufacturers instructions. The ice machine must be maintained free of mold. Correct By: 02-Apr-2015

Observation: Liver based mousse products were found in the display cooler without a date mark. The factory packages were opened. Opened factory packages of soft cheeses like brie and camembert were not dated. Opened factory packages of diced cooked chicken pieces and shredded chicken were not dated. House made chicken salad were not date marked. The operator knew when the packages of liver mousse and soft cheese had been opened. The operator knew when the chicken for the salads were opened and made. The operator did not know when the diced chicken and shredded chicken were opened.
Correction: Adjust method or procedures so that all these ready-to-eat potentially hazardous foods (time/temperature control for safety food), are provided with date marking or an effective method of identifying when a food item has exceeded 7-days. The operator provided date marks indicating the remaining number of days on the packages where the open date was known. The operator discarded the chicken packages where the open date was not known.

Observation: The can opener has an accumulation of old food debris.
Correction: Clean and sanitize the can opener at least every four hours if used. Correct By: 01-Apr-2015

Correction: Provide & post an original State of Wisconsin food manager's certificate. Correct By: 30-Apr-2015

Observation: Shelves in the walk in cooler and the reach-in cooler in the kitchen area are moldy and dirty.
Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment. Correct By: 08-Apr-2015
