    Observation: Tuna and whitefish were found in the prep cooler in a thawed state, still in the vacuum sealed packages.
    Correction: Fish must remain frozen when in a vacuum sealed package. Remove fish from the vacuum sealed package or re-introduce air into the package immediately when thawed. Correct By: 08-Jan-2016

    Observation: The soda guns/drip cups and the fountain dispenser heads have an accumulation of slime.
    Correction: Clean and sanitize these areas. These areas must be maintained free of accumulations of slime. Correct By: 09-Jan-2016

    Observation: Ice making water is circulating against moldy surfaces inside of the ice machine.

    Observation: Cooked and cooled baked potatoes, portioned sandwich meats were not date marked.

For example, for hamburgers and steaks that are cooked to the consumer's liking, provide a symbol next to these items on the menu that leads to a footnote that states "Hamburgers and steaks ordered rare, medium rare, or pink in the middle might be undercooked.     Observation: Thawed fish found in vacuum packages. The label on the package states that fish must be removed from the package and thawed.
    Correction: Fish in the vacuum packages must be opened immediately when thawing to introduce oxygen into the package and avoid the growth of C. botulinum bacteria. Correct By: 03-Sep-2014

    Observation: Ice making water is coming into contact with moldy surfaces inside of the ice machine.
    Correction: The ice machine must be kept free of mold. Empty, disassemble, clean, and sanitize the ice machine according to the manufacturers instructions. Correct By: 04-Sep-2014

    Observation: Soda beverages are coming into contact with moldy surfaces in the nozzles of the soda fountain system and in the soda guns and drip cups at the bar.

    Observation: The mechanical dishwasher is not sanitizing the food equipment. The gauge does not register 180F final rinse water. The 160F thermolabel does not indicate 160F at the equipment surface.
    Correction: Repair the dishwasher so that it shows 180 to 195F water in the final rinse and so it meets 160F on the untensil surface as indicated by the thermolabel. The operator set up the three compartment sink with a chlorine bleach water solution (100 ppm) at the time of this inspection and will sanitize all equipment manually until the dishwasher is repaired.   • FOOD CONTACT SURFACES - SOILED
    Observation: The can opener was visibly dirty.
    Correction: Clean and sanitize the can opener at least every four hours if used. Correct By: 03-Sep-2014
  • HAND DRYING PROVISION (corrected on site)
    Observation: No single-use toweling or other hand drying device available at handwashing sink for hand drying.
    Correction: Provide single-use toweling or other approved devices at handwashing sink to facilitate proper handwashing. Operator found a roll of toweling at the time of this inspection. The toweling must be placed in a dispenser. Correct By: 03-Sep-2014
  • PLUMBING - SYSTEM MAINTAINED IN GOOD REPAIR
    Observation: The bar's ice bin drain is not maintained in good repair. Slime is blocking the drain and the waste water is overflowing into a bus tub below the drain.
    Correction: The plumbing system shall be maintained in good repair. Clean out the plumbing and ensure proper drainage of wastewater. Correct By: 10-Sep-2014

    Observation: The walk in cooler light fixture is not shielded.
