Correction: A written consumer advisory that both discloses which food items are undercooked, and reminds the consumer that consuming the undercooked items is an increased health risk. For example, place an asterisk next to the cooked to order egg items on the menu that leads to a footnote that states "Eggs ordered sunny-side up, over-easy, or otherwise soft cooked might be undercooked. Whether dining out or preparing food at home, consuming undercooked eggs, meat, poultry, seafood, or fish may increase your risk of foodeborne illness.

Observation: A pan of raw ground beef was stored over potatoes in the prep line cooler with clear doors.
Correction: Reorganize storage so that ready-to-eat foods are stored above raw animal food and raw vegetables. Discard ready-to-eat foods that show evidence of contamination.
- Critical: FOOD CONTACT WITH EQUIPMENT AND UTENSILS
Observation: Ice making water is coming into contact with slightly moldy surfaces inside of the ice machine.
Correction: Maintain the ice machine free of mold. Empty, disassemble, clean, and sanitize the ice machine according to the manufacturer's instructions. Correct By: 26-Nov-2014
- Critical: PHF/TCS, COLD HOLDING (corrected on site)
Observation: A bulk portion of ham (in the prep cooler with the clear doors) was found at 58F in the center. The operator stated that the ham was sitting out at ambient kitchen temperature for the breakfast time (this inspection was conducted at 12:30PM)
Correction: All foods must be maintained at 41F or less. This food item must be discarded at this inspection. If foods must be set out for accessibility, a food prep refrigerator will be required. Foods cannot be temperature abused and then returned to the refrigerator. The ham was discarded at this inspection.

Observation: Clean knives were being stored in the crevice between the sink backsplash and the wall (in the back kitchen prep area). The crevice is not a clean area.

Correction: Maintain cold potentially hazardous foods at or below 41°F
- VARIANCE OBTAINED FOR SPECIAL PROCESSES
Observation: There is no documentation of an approved variance for pickling herring and beets.
Correction: Submit a variance application to the regulatory authority for submission to the State for review. If approved, a copy of the approved variance shall be kept at the establishment and be subject to review by the inspector upon request. Discontinue current process until a variance can be obtained from the State.

Correction: Provide a consumer advisory for animal foods served in a raw or undercooked condition.
- ADEQUATE EQUIPMENT FOR TEMPERATURE CONTROL
Observation: Drawer cooler and steak cooler found at 42 and is not sufficient in capacity to hold foods at at 41 F or below.
Correction: Provide equipment in sufficient capacity to hold food at proper temperature. Contact repair company if cooler does not hold temp. I will follow up on temps in January, maintain a temperature chart on all coolers. Reduce food inventory or provide additional approved equipment.
- FOOD CONTAMINATION PREVENTED FROM PREMISE - NOT EXPOSED TO SPLASH AND DUST
Observation: The food items stored in WIC is not covered.
Correction: Store food in areas not exposed to splash, dust, or other contamination.

Observation: The roast chickenis stored on the floor in the WIC.
Correction: Store all food items 6 inches above the floor.
