Permit expires 4/30/16. The repeat violations must be addressed. It has been a year since they were documented to be a violation. Call me for a reinspection. PIC was able to list symptoms for workers to report and where to find the big 5 enteric diseases that require exclusion.
  • Handwashing Lavatory, Water Temperature, and Flow (repeated violation)
    Observation: Water from the handwashing sink at the front and women's restroom was measured at a temperature less than 100°F.(97 F and 94 F respectively)
    Correction: Make necessary adjustments to valves and lines serving the handsink to provide water of at least 100°F to allow more effective removal of fatty soils encountered in kitchens.
  • Plumbing System Maintained in Good Repair
    Observation: Faucet at front prep sink is leaking. There appears to be a leak from waste pipe under prep sink.
    Correction: Repair
  • Physical Facilities in Good Repair (repeated violation)
    Observation: Surface of floor in back area is in poor repair. Finish has been compromised. Part of the floor seam has been taped by the ice machine.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
  • Physical Facilities - Cleaning Frequency and Restrictions
    Observation: Caulking behind front prep sink is stained.
    Correction: Remove old caulking and replace with new.
  • Dressing Rooms - Using Dressing Rooms and Lockers
    Observation: Employees are not using the designated area or lockers provided. Purses found on back supply shelves.
    Correction: Personal belongings are to be kept in lockers and not on food supply shelves..
  • Critical: Pests-Controlling Pests*
    Observation: Observed methods are not being used to control pests. Numerous drain flies seen up front.
    Correction: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by using methods such as trapping devices or other methods as required. The last noted visit by PCO appears to have been 1/25/16. Please seek their advice.

    Observation: Permit is not posted in a place where it is readily observable by the public.
    Correction: Post the permit in a place where it is readily observable by the public transacting business with the establishment.

Noe: a few drain flies observed up front. PIC stated situation has been improved. Monitor. Permit issued. Post in public view. PIC knew foodborne illness symptoms.
  • Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils
    Observation: Deep sink cabinet is soiled. Interior of 2 door freezer is dirty, cabinet where urn cleaner is stored found soiled.
    Correction: Clean all thoroughly.
  • Handwashing Lavatory, Water Temperature, and Flow
    Observation: Handsink up front had 87 degree water. Handsink in back was 94.4 degrees.
    Correction: Handsinks to have 100 degree water available. Adjust temperature.
  • Physical Facilities in Good Repair
    Observation: Floor in back room especially by dishmachine has surface coat deterioration.
    Correction: Floor to be easily cleanable.

Some drain flies noted in kitchen. Ask Pest Control officer for advice. Wiping cloth had no residual. Change. Employee health policy available.

    Observation: Open drinks found on food shelf.
    Correction: Personal drinks to have lids and straws or place in designated area in kitchen. If drinking from open cups workers are then to wash hands prior to returning to work. PIC discarded cups.
