    Observation: Raw food of animal origin holding in a manner that may cause cross contamination of to ready-to-eat food (RTE). Raw eggs over half and half.
    Correction: Separate raw foods during storage, preparation, holding, and display from raw RTE food including other raw food such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.
  • Outer Openings - Protected (corrected on site)
    Observation: Outer opening of the food establishment is not protected against entry of insects and rodents. Front door left open.
    Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.

    Observation: Soap was not provided at the hand washing lavatory in the back kitchen handsink.
    Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.

Generally, facility appeared to be clean and well ordered. Sanitizer set up in 3rd compartment of sink at proper concentration-chlorine at 75ppm. Recoomended that operator maintain written log of food temperatures throughout the flow of food especially after prep and at point of delivery. Also recommended that barrier be installed to right of main kitchen handsink.

    Correction: Have the license holder require employees or applicants report to the person in charge any information about their health and activities if they are diagnosed with or had a past illness from Salmonella Typhi, Shigella spp., Escherichia coli O157:H7, Hepatitis A virus or Norovirus

    Correction: Have the license holder require employees or applicants report to the person in charge if they are experiencing the following symptoms or conditions: (1) diarrhea, fever, vomiting, jaundice, or sore throat with fever. (2) A lesion containing pus such as a boil or infected wound that is open or draining and is on the hands or wrists, unless an impermeable cover such as a finger cot or stall protects the lesion and a single-use glove is worn over by an impermeable cover or on other parts of the body, unless the lesions covered by a dry durable, tight-fitting bandage.

    Observation: The nonfood contact surface of the interior of upright freezer has frost buildup. It is not corrosion resistant, nonabsorbent, and/or smooth.
    Correction: Nonfood contact equipment that is exposed to splash, spillage, food soiling, or requires frequent cleaning must be smooth, durable, and nonabsorbent. Clean more frequently as frost can contain Listeria, a microorganism that can cause foodborne illness.
  • Equipment, Utensils, Linens, and Single-Service and Single-Use Articles
    Observation: Clean knives stored in knife block that appeared very difficult to clean.
    Correction: Operator agreed not to use this knife block that was not cleanable. Store all utensils in a cleanable location.
