The purpose of today's visit was to conduct a routine inspection. Note to owner: A Certified Food Manager (CFM) must be onsite at ALL times of preparation and hours of operation. It is unlawful to operate a food establishment unless under the direct supervision of a CFM. The CFM has many responsibilities, one of which is to make sure that food is being prepared and served safely to prevent foodborne illness. The lack of a CFM is grounds for closure. EHS provided the CFM once he arrived a copy of the CFM guidelines. Inspection time reflects waiting for a CFM to arrive and remain in the facility in order to help prevent a facility closure.
  • Food Storage / Preventing Contamination
    Observation: Food stored on the floor and/or food stored less than 6 inches off the floor.-------Observed a box of strawberries and container of yogurt stored on the floor in the Walkin Cooler.
    Correction: Food shall be protected from contamination by storing the food at least 6 inches off the floor on approved shelving units or dunnage racks. Milk crates, soda crates, or bread racks are not suitable for food storage.
  • Temp Meas Devices for Ambient Air Location
    Observation: The temperature measuring device in the following mechanically refrigerated unit was not properly located in the warmest part of the unit:-------both True 1DR prep cooler topping stations.
    Correction: In a mechanically refrigerated unit the sensor of a temperature measuring device shall be located to measure the air temperature of a simulated product temperature in the warmest part of the refrigerator. Store the thermometer near the door of the unit.
  • Permit to Operate Required, Must be Posted, Must be Valid
    Observation: The establishment has not posted the 2015 permit to operate in a conspicuous location.
    Correction: A valid permit shall be posted in every food establishment in a conspicuous place in the public view. Food establishment permits are issued for a calendar year and expire on December 31 regardless of the month of issue.
  • Critical: Certified Food Manager/Presence Required (corrected on site)
    Observation: There is no Certified Food Manager present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment. TWO LCFMS ARRIVED AT THE END OF INSPECTION WHEN EHS WAS PREPARING TO PRINT OUT INSPECTION REPORT. EHS DISCUSSED WITH LCFMS THAT ONE HAD TO REMAIN ON DUTY UNTIL THE FACILITY CLOSED FOR THE DAY OR THE FACILITY WOULD NEED TO VOLUNTARILY CLOSE.
  • Handwashing Sink / Used for Hand Wash Only (corrected on site)
    Observation: Observed the handsink in the back being used to store a container fill of utensils.
    Correction: Handsinks are to be used only for washing hands to encourage proper hand washing and to prevent contamination of cleaned hands. RELOCATED.
07/09/2015Routine
**Walk in cooler has been repaired. Approval is hereby granted for the issuance of a Health Department Permit. This inspection report shall serve as your permit until the official permit is received. All outstanding violations are to be corrected no later than the time of your first routine inspection (approximately 30 days). Equipment additions/replacement/changes are not allowed without Health Department approval.**
Note the following recommendations:
It is recommended that more than one person on staff be a Certified Food Manager. A Certified Food Manager must be on the premises during ALL food preparation and service.
It is recommended that a monthly pest control program be implemented. Maintain records of pest control treatments for Health Department review.

No violation noted during this evaluation.
04/14/2015Pre-Opening
Facility does not have enough refrigeration to store PHFs. Approval for a Health Department Permit to operate is denied at this time. Review the cited items above. All items must be corrected as recommended at the time of the next pre-opening inspection. Call the Health Department at 703-246-2444 in order to arrange another inspection.
  • Equipment / Good Repair / Operation
    Observation: The following refrigeration unit is not operating properly and is unable to maintain cold food at or below 41°F: WALK IN COOLER
    Correction: Repair/adjust the unit so it is able to maintain foods at or below 41°F. Do not use the unit to store potentially hazardous foods while it is not operating properly.
  • Handwashing Cleanser, Available at Handwashing Facilities
    Observation: Observed that hand soap was not provided at handwashing sink near prep sink.
    Correction: Provide hand soap at each hand sink to allow employees to properly wash their hands.
  • Hand Drying Provision / Heated Air-Hand Drying Device
    Observation: Observation: No hand drying provision (paper towels) at hand near the prep sink.
    Correction: Provide paper towels at hand sink to allow employees to properly dry their hands after handwashing.
