  • Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site)
    Observation: TCS food in drawer refrigeration unit was >41°F. Pasta in 6 drawers was 45-52°F. Cooked chicken portioned in bags over the cooling line was >41°F. Sausage cooked about 3 hours ago found at 61°F in prep top of one unit - TCS food next to it also >41°F.
    Correction: PIC moved the pasta (in unit for <4 hours) to another refrigerator to cool immediately to <42°F. Service tech was called and will be here to service the unit today. Chicken was relocated and employees told to avoid overstocking the prep tops. Both the sausage and other TCS food was moved to another refrigerator to cool to <42°F. Train employees to use walk-in coolers to cool all foods to <42°F before putting them in the prep coolers or other units on the line.

03/04/2015Follow-up
Investigation after receiving reports of possible foodborne illness by persons that attended a banquet here on 02-28-2015 and persons that ate lunch here (separate from the banquet) that same afternoon.
Food holding temperatures and employee handwashing and glove use practices were good during this inspection. Employees are aware of the requirement to report illnesses and symptoms of illness (vomiting, diarrhea, fever with a sore throat and jaundice) and lesions on the hands/arms to the PIC. The dishmachine is not working properly and is not reaching sanitizing temperatures. The booster heater was off when I ran the first test of the machine. The employee turned on the booster heater. After allowing time for the unit to heat up another test was run - the temperature of the wash cycle was dropping below the required 160°F during the cycle. The machine was taken out of service and the technician was called for service. PIC agreed to use 3 part sink or glass washer to sanitize wares until the machine is fixed. Wares washed earlier today will be rewashed to make sure they are sanitized.

  • Critical: Equipment and Utensils - Hot Water and Chemical Sanitizing* (corrected on site) (repeated violation)
    Observation: Dishmachine wash and rinse temps were not hot enough to sanitize wares.
    Correction: Machine was taken out of service. Technician arrived and is working on machine. 3 part sink can be used until machine is repaired. Wares washed earlier today will be rewashed to make sure they are sanitized.

Discussed safety concerns of thawing vacuum packaged salmon in the sealed pkg. C botulism can grow and produce toxin in this environment - open salmon/fish vacuum sealed bags when the fish is removed from the freezer. Water standing behind dish machine and residue build up on wall under the machine need to be corrected. Also need to get documentation that farm raised salmon is pellet fed since it is offered undercooked.

    Observation: TCS foods in Pasta 2 reach in cooler found >41°F. Unit is being repaired. PIC states that foods in the unit overnight were discarded. Also had eleveated temps in RIC near dessert area.
    Correction: PIC agreed to implement a time control policy for the TCS foods in this unit. Written policy was devised and TCS foods will be discarded at 4 PM if not discarded.
  • Hand Drying Provision (corrected on site)
    Observation: No paper towels at one handsink on line.
    Correction: Paper towel dispenser was refilled.

PIC knew illnesses and symptoms that employees must report. Employees use thermometers and know temperature requirements. Good employee handwash and glove use practices. Good cooling practices.
PIC agreed to find out if steaks are whole muscle beef (they are included in the consumer advisory), if the farm raised salmon is pellet fed or frozen to destroy pathogenic parasites & find out if Salmon packages are oxygen permeable,

    Correction: Remove labels to ensure containers are cleaned and sanitized after use before reuse.

    Observation: Interior of 1 ice machine is not clean.
    Correction: Ice machine was labeled with "do not use" sign and will be cleaned tonight.
  • Light Bulbs Protective Shielding
    Observation: Light fixtures on cook line are not shielded.
    Correction: Provide shields.
  • Physical Facilities in Good Repair
    Observation: Floor covering was removed. There is some residual adhesive on the floor which doesn't appear to be interfering with cleaning except in a few areas where the adhesive is loose.
    Correction: Remove loose adhesive.
