Observation: Rinse guage on the dish machine is not working properly.
Correction: Repair the gauge to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. Observation: Surfaces of the metal ice machine slide trays was in contact with non-potentially hazardous food items that were observed soiled with accumulations of grime and debris. (wet rust and gunk).
Correction: Clean the surface of slide trays at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications.

Observation: Single service items in the storeroom were observed stored unprotected on the floor ( keep up off the floor).
Correction: Store up off the floor in the original protective package to protect from contamination until used.

Observation: Trash and litter were observed adjacent to the refuse container outside the facility. (recycle cardboard dumpster).
Correction: The refuse container storage area and grounds adjacent to the container are to be maintained clean and sanitary.

Observation: Metal trim around the door frame inside of the walk-in cooler - (pulling away from the trim...very sharp tin) is not maintained in good repair

Observation: Under the broiler cooler drawer and the middle deli-top cold holding at improper temperatures
Correction: Relocate food to a refrigeration unit that is capable of maintaining food storage at 41°F or below. (foods in both units were iced down and the refrigeration man was called to come out and repair these units.
