    Observation: Observed several dead and live roaches throughout the kitchen.
    Correction: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by using methods such as trapping devices or other methods as required. Have pest control company treat every 10-14 days until the problem is resolved, make sure bait stations are being used as well as the glue traps and aerosol.

The day care and the church are both responsible for the cleanliness of the kitchen, the permit will not be reissued until the roaches are under control. The day care currently brings most of its food from other restaurants to serve the children (Kroger & Martin's hot bar).
  • Utensils - In-Use - Between-Use Storage
    Observation: Observed utensils stored where they were all thrown into a bin.
    Correction: Have utensils placed in the bin so that the handles all face the same way and the food contact surfaces are not being contaminated when grabbing utensils.
  • Critical: Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils*
    Observation: Observed plastic dishes soiled with residue from labels.
    Correction: Make sure the residue is removed completely from all the dishes to prevent contamination of other dishes and so that it will not hold bacteria.
  • Non-Food Contact Surfaces
    Observation: Observed the bottom of the freezer and the ovens both in need of cleaning.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.
  • Handwashing Cleanser - Availability (corrected on site)
    Observation: Soap was not provided at the hand washing lavatory in the kitchen.
    Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.
  • Hand Drying Provision (corrected on site)
    Observation: No disposable towels were provided at the hand washing lavatory in the kitchen.
    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials

    Observation: Observed the floors and walls throughout the kitchen (including vents and exposed pipes) in need of cleaning.
    Correction: Clean kitchen thoroughly, especially under and behind the equipment (Day care and Church equipment).

    Observation: Observed several dead and live roaches throughout the kitchen.

    Observation: Observed several items in the kitchen that need to be removed to prevent harborage conditions for pests.
    Correction: Remove all items that are not being used from the kitchen.

    Observation: Observed pesticides in the kitchen not suitable for use.
    Correction: Pesticides can only be applied by a licensed pest control operator. Remove the can of roach killer from the kitchen.

Observed several roaches in facility, have pest control treat about every 10 days to resolve the issue. No traps were observed in the kitchen. Kitchen could also use some cleaning, this will also help prevent roaches. Clean equipment good, the walls, the floors, and under/behind equipment.

    Observation: Observed raw shell eggs stored over RTE foods.
    Correction: Move eggs to the bottom shelf of the refrigerator.
  • Handwashing Cleanser - Availability (corrected on site)
    Observation: No soap was available at the hand sink.
    Correction: Provided soap. Get more soap other than the dish washing soap for the hand sink.

    Observation: PIC was not able to explain how to set up the 3 compartment sink correctly. PIC also did not know the proper temperature to reheat foods to.
    Correction: Provided with information about food temperatures and food safety.
  • Critical: Food - Potentially Hazardous Food - Cold Holding* (corrected on site)
    Observation: Observed food in refrigerator greater than 41 degrees.
    Correction: The refrigerator does not work properly, and is not holding food below 41 degrees. Refrigerator needs to be fixed or replaced. Do not store any cold foods in the refrigerator until it is fixed. Milk was discarded.
  • Physical Facilities in Good Repair (repeated violation)
    Observation: No base coving in the kitchen.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities. Coving is required to seal the joint between the floor and the wall in the kitchen.
  • Critical: Animals - Prohibiting Animals*
    Observation: Observed live and dead roaches in the facility.
    Correction: Prohibit all live animals on the premises of the food establishment. Animals carry disease-causing organisms and can transmit pathogens to humans through direct and/or indirect contamination of food and food-contact surfaces. Animals shed hair continuously and may deposit liquid or fecal waste, creating the need for vigilance and more frequent and rigorous cleaning effort. Contacted pest control company to come and treat for roaches.
