This visit was made to follow up to the routine food safety evaluation conducted on January 11 2016. The following actions have been taken:
- More regular cleaning is taking place in the dry storage areas and along the walls and there is active pest control methods in place. Please check the traps regularly to ensure timely removal of any pests. Thank you for taking these actions
Please note the following issue that requires attention:
- Please remove the unused equipment in the storage area on the side of the kitchen. It is currently being used to store old equipment and it is full of equipment which is preventing regular cleaning and providing harborage conditions for pests.

  • Plumbing / Maintained in Good Repair (repeated violation)
    Observation: Hot water faucet at handwashing sink in mens restroom/locker room is not functioning properly.
    Correction: A plumbing system shall be maintained in good repair.
  • Controlling Pests by Routine Inspection of Premises (repeated violation)
    Observation: Droppings observed on floors near storage room for old unused equipment and in dry storage room floor.
    Correction: Insects and other pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. Routinely inspect the premises to control the presence of pests on the premises. Please have kitchen area inspected and cleaned more frequently for droppings and other evidence of pests, and it is reccommended that pest control methods are increased.
  • Controlling Pests by Eliminating Harborage Conditions
    Observation: The storage area on the side of the kitchen is being used to store a lot of unused equipment which is providing harborage conditions for pests.
    Correction: Insects and other pests are capable of transmitting disease to people by contaminating food and food-contact surfaces. Eliminate harborage conditions to control and prevent the presence of pests on the premises. Please remove any unused equipment from the storage area to allow for regular cleaning of this area. The number of pieces of equipment being stored in this room prevents thorough inspection and cleaning.

This visit was made to conduct a routine safety evaluation. The Person in Charge was very helpful during the visit and the food employees were knowledgeable on foodborne illnesses and symptoms. Thank you. The following issues require attention:
1. Observed evidence of rodent activity. Please ensure that the dry storage areas and areas along the wall are cleaned frequently and please increase the frequency of pest control methods. A follow up visit will be conducted in approximately 1 month

  • Utensils That are In-Use / Between-Use Storage (corrected on site)
    Observation: Observed serving utensils stored on disposable trays in between serving periods.
    Correction: During pauses in food preparation or dispensing, food utensils shall be stored in one of the following manners: 1) in the food with their handles above the top of the food and the container, 2) in food that is not potentially hazardous (time/temperature control for safe food) with their handles above the top of the food within containers or equipment that can be closed, 3) on a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food contact surface of the food preparation table or cooking equipment are cleaned and sanitized, 4) in running water of sufficient velocity to flush particulates to the drain, 5) in a clean, protected location if the utensils are used only with a food that is NOT potentially hazardous (time/temperature control for safe food), or 6) in a container of water if the water is maintained at a temperature of at least 135°F and the container is cleaned frequently. Manager moved the utensils to store in foods in between lunch periods.

Follow-up done in office. Spoke with manager who stated parts for the walk in door have been ordered (invoice attached). During last inspection facility replaced new milk boxes that were not working with older milk boxes that were at proper temperature. Glass door refrigerator on the line has been repaired. Walk in door is to be repaired as soon as parts come in. Last day of school is Friday 6/19/15.

    Correction: Potentially hazardous foods (time/temperature control for safety food) shall be held cold at a temperature of 41°F or below.

    Observation: The following mechanical refrigeration unit is not currently operating as required to cold hold food at a temperature of 41°F or less: both new milk boxes (may be under defrost), 2 door glass refri. on the line (TCS foods removed)
    Correction: Refrigerators shall be capable of holding foods at a temperature of 41°F or below. Repair the unit(s) immediately. Foods exposed to temperature abuse for over 2 hours shall be discarded, otherwise, the foods shall be stored in properly operating refrigerators.

    Correction: Windows in the establishment must be tight-fitting and remain closed. If windows are left open for ventilation, they must have mesh screens as specified under 6-202.15 to prevent the entry of insects and rodents. .

This visit was made to conduct a routine food safety evaluation.
Excellent job with hand washing and glove use.
Great improvement in the mouse infestation. Employees and manager reported that while they still catch a few mice here and there, they are not getting into the food anymore. Thus, they have started using the dry storage room again. There was one dead mouse in a sticky trap and one sticky trap with bait in the middle, which is not the correct way to set up the bait. I will contact the pest control manager to ensure that the traps they set are effective. Person in charge in the kitchen agreed to remove dead mice as soon as they find them, and to check everyday.

This visit was made to attend a meeting with members of school staff, members from the school board, the pest control company (Professional Pest Control) and the Health Department in regards to the mouse infestation at the school. The plan moving forward is to have the pest control company do a major treatment of the whole school over this weekend. This will be in conjunction with school maintenance and/or facilities conducting an evaluation of the school for points of entry, and sealing them. The principal also plans to make all school staff aware that food must be sealed tightly if not removed completely, and that issues must be logged in the school's pest control log books. A follow-up will be conducted in another 2 weeks to see that conditions have improved after this collaborative effort.
11/14/14 Update: Debra Yap reports that 2 weekends of eradication were completed, and they are still waiting on Professional Pest for an evaluation of points of entry.

This visit was made to conduct a follow-up inspection regarding mouse infestation, particularly as it affects the cafeteria.
I received an e-mail from Mr. Mulcahy claiming that there was some pest control work that was going to be done over the weekend of the 18th of October. This work was not completed according to the maintenance engineer, Mr. Horton. The compost outside is now removed daily, and the bins are also cleaned daily. They are checking the traps and removing dead mice from the kitchen every other day. I instructed them to perform this task everyday. Kitchen staff is keeping as much food in the walk-in as possible to avoid contamination of food in the dry storage room. Contaminated packages are discarded. There was still mouse droppings on the floor and on some boxes that were discarded. The manager reported that if food boxes are left in the storage room for more than a few hours, they may be contaminated by the mice. More remediation action is needed at this time.
For now, the school workers should try to keep the food racks that won't fit in the walk-in outside of the storage room and/or wrapped with plastic wrap at night to avoid contamination. Continue to discard contaminated boxes and food or food contact items. Clean droppings and discard dead mice daily. I will contact Debra Yap with ACPS to try to get this issue resolved. Further instruction will be communicated via e-mail or phone if necessary.

This visit was made to conduct a routine food safety evaluation.
There is a serious mouse infestation in the kitchen which needs to be addressed and abated as soon as possible. There were mouse droppings observed on the floor of the storage room in the kitchen, as well as all over the boxes of cereal.
The manager reported that she is trying to keep foods in the walk-in as much as possible. Boxes that get contaminated with mouse droppings are wiped with sanitizer solution be workers with gloved hands. The worker then washes hands and changes gloves before handling the prepackaged cereals inside the box. She also cleans the room everyday of mouse droppings. I recommend cleaning mouse droppings from all surfaces and boxes first thing every morning until the problem is resolved. Wash hands after, and wear gloves while cleaning mouse droppings.
The manager was able to produce a pest control service report from yesterday (9/25/14) that reports treatment with snap traps and glue boards from Professional Pest Solutions. However, the report does not explicitly state that mice are the target pest. There were some traps with poison bate in the storage room, and snap traps throughout other areas of the kitchen. That being said, there is an area in the corner where some piped come through the wall that is not sealed properly, and may be an entrance point for mice and other pests when the door to the room is shut.
Pictures of the hole and the droppings were taken, and will be attached to this report. The report and pictures will also be sent to the managers' supervisor, Ronjanica Edwards at ronjanica.edwards@acps.k12.va.vs.
A follow-up inspection will be conducted in 14 days to check on the progress of the mouse abatement.

This visit was made to conduct a routine food safety evaluation.
1. Monitor temperature of milk in milk coolers. If the temperature rises above 41 degrees while left open for service, move milk to walk-in or walk-in freezer to cool as soon as possible. Temperature logs show that the temperature is usually at or below 41 degrees by the next morning. Do not use if the temperature is above 41 degrees F. Try to keep these units closed as much as possible before and after service.
2. Remember to ALWAYS wash hands before putting on gloves to begin a new task.

This visit was conducted to follow up on a suspected outbreak of norovirus at the school.
1. No sick team members. - Team briefed on symptoms and the need to stay at home if they are suffering with them.
2. Hand washing requirements reinforced.
3. Kitchen is currently using quat sanitizer. Strongly advised to switch to bleach as this is more effective at controlling norovirus. I will speak to the school food director to investigate this.
