    Observation: Observed several items in the food prep area in need of cleaning (outside of grill, outsides of deep fryer, rolling tables on cook line, interior of microwave oven, 2 door fry refrigeration unit, gaskets to 2 door fry refrigeration unit, outside of "Prince Castle" bun roller, 2 door hot hold box, tops of food containers on the prep line).
    Correction: Clean.

    Correction: Keep lid/doors closed. Lid was closed.

    Observation: Observed cracked light shields in the walk-in vault/walk-in freezer.
    Correction: Replace the shields.

    Observation: Observed 4.23 foot candles of lighting in the walk-in vault

    Correction: PIC spoke to worker and hands were washed.

    Observation: Shredded cheese cold holding at improper temperatures Cheese was 53 F.
    Correction: PIC discarded. You may decide to place on 6 hour time control if you cannot maintain 41 F or below.

Most items were corrected. Consider caulking open seam inside walk in refrigerator along floor wall junction. Cement wall in electric panel room has peeling paint. Consider repainting walls in panel room. Drain pipe in drive thru has been a constant issue. It is difficult to reach to keep clean. Therefore, it seldom gets cleaned. Consider removing panel in front of ice bin for easier access. Permit issued.
  • Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils (repeated violation)
    Observation: Drip trays to flat top have build up grease, Bunn support rack is soiled. Area under supply shelf is dirty.
    Correction: Clean these areas.
  • Physical Facilities - Cleaning Frequency and Restrictions
    Observation: Floor drain under drive thru ice bin is dirty and pipe from drink station has build up. Crumbs found along electric panels.
    Correction: Clean. Put this on your cleaning schedule. Brush crumbs from panel.

A thirty day permit is issued to give you time to correct the above items that still require attention. There will be no additional time given after this one time extension.
Recaulk seam on front of fryer. Time control tracking for shredded cheese indicated a 120 hour hold time on the table. This is incorrect. Have correct time on print out.
2350ii Water leak in mop sink area. Repair.

    Observation: Caulking around outside of fryers has deteriorated.
    Correction: Remove old caulking. Area to be smooth and cleanable.

    Observation: The following items were found dirty: warmer cabinet and latches, Bunn support, Traulsen freezer gaskets, edges to most equipment, sandwich board, toaster, broiler, between flat tops, flat top wheels, bacon presses, drip tray, chicken drip tray in walk in, dunnage racks, ice chute.
    Correction: Clean. Pay attention to detailed cleaning.

Permit expires 1/31/14. Above items to be corrected. Call for reinspection. Sausage patties were found in a closed container inside walk in at 57 F. You must cool foods to 70 F within 2 hours then to 41 F in 4 more hours. PIC did not know how long procedure takes. Management to write a policy for cooling this item. Overall sanitation is lacking. A lot of cleaning is needed. Provide a trashcan near handsinks. Drain from soda/ice bin near drive thru is once again an issue. This area is neglected. Management to correct this.

    Observation: Hot dogs found at 108 F.
    Correction: Maintain 135 F or higher. PIC discarded.

    Observation: Creamers found at 48 F. Apparently these were on counter during breakfast,
    Correction: PIC discarded. Ensure frozen crock is frozen before use.
  • Critical: Date Marking - Ready-to-Eat - Potentially Hazardous Food - Date Marking* (corrected on site)
    Observation: No date marking found on steak product or on omelette mixture.
    Correction: TCS foods , RTE and kept for more than 24 hours must have a date marked on container. PIC placed appropriate date.

    Observation: No time was marked for those foods on time control on the sandwich board.
    Correction: When placing food on time control you must track the time on your chart. PIC discarded blue cheese and changed out other pans.

Manager knew symptoms associated with foodborne diseases. Review these and the Big 5 enteric diseases with managers. Other observations: Cleaning or lack of cleaning is a continued problem. Slime growing from drive thru drain. Grease build up under and around fryers. Step up cleaning practices. Provide trash can near handsink. Repair ice machine top - it is open at top. Clean top of ice machine. Next inspection is routine and I expect restaurant to have improved sanitation before permit reissuance.

    Correction: Supply policy. Cheese was given a 48 hour time. Shredded cheese 5 days. New cheese placed on line but old cheese still on line. Managers to have a clear system.

Follow up on cleaning issues. Power washing has been done. Area where electrical box under pass thru has been cleaned. Continue to keep it looking like this. Area under and behind fryers were cleaned. Continue to clean nightly or build up will occur again.

Follow up on cleaning items. New pipe installed from front ice bin. Keep it clean. New door to power cabinet is beginning to have grease built up. This to be cleaned daily. The door to electrical box still has grease. This to be cleaned. Area under fryer has a tremendous amount of grease. Side of refrigerator also has build up. This entire area needs thorough cleaning, including wheels of fryers. Any wires under cabinet that are not hooked to anything should be removed or thoroughly cleaned. Call when complete.

Meeting with employee to follow up on cleaning issues from previous inspections. Area by circuit box still requires cleaning to remove grease. Some cleaning has been done. A full door has been installed to keep further accumulation of grease from happening. Area under fryer cabinets still has grease build up. Clean more thoroughly. PVC pipe from drive thru ice bin has slime coming from it. Clean thoroughly or replace. Clean floor under cabinet. District manager agreed to bring in a professional to clean above areas this week. He will call when completed. Daily cleaning is essential.

Other observations: Area by circuit box under French fry bin is extremely greasy. This must be thoroughly cleaned. Repeat from 11/29/12 and 1/6/13. Fryer cabinets are greasy. Clean thoroughly. Floor and floor drain and pipes to be cleaned under drive thru area. Leak from pipe under drive thru beverage area. Permit was not posted. Post permit in public view. Step up cleaning practices.

Follow up to ensure area under French fry bin was cleaned. Area still needs to be thoroughly cleaned. Discussed fish prep. Fish frozen to thaw in refrigerator (12 hour thaw). Placed in broiled. Cooked to 155 degrees. It appears product is raw. Tan tongs are used for this product. Cooked to order.

02/15/2013Follow-up
Many items have been corrected. There are a few that still require attention. Grout has been laid down in needed areas. New FRP walls installed in mop area/water heater area. Some new container lids have been purchased. Chicken wash was 32 degrees with the help of ice. Mop sink floor has been re-concreted. New bait box on premises. Establishment requires a continued cleaning schedule to be followed. Permit issued.

    Observation: Meat presses still have build up that was not removed.
    Correction: Replacement may be necessary.

    Observation: External thermometer attached to undercounter refrigerator is dirty. Breaker cover area near French fry pass thru is covered with grease. Fryer cabinets have been cleaned but require further attention and constant attention to keep in a sanitary condition.
    Correction: If it does not work, remove it. Ask maintenance about advice to clean around electrical lines

    Observation: Tile is missing from floor drain under beverage counter at drive thru.
    Correction: Repair and make area cleanable. Continue to clean area.
