This is a reinspection for the presence/abatement of an ant problem near the soda fountain. Live activity was seen around the soda fountain. Per a service report from PCO the perimeter of the facility was treated for ants on 7/27/15. The inside of the facility was locked/tech was unable to treat the facility. Complaint on 23-Jul-2015 : This visit is due to a complaint in regards to ants being seen especially around the soda fountain station. Upon inspection of the facility, ant activity was seen underneath the counter near the soda fountain/window sill/outside. Per a service report from PCO dated 6/15/15 ants were traveling inside from exterior flower beds/recommend the exterior perimeter to be treated for ants. This visit is due to a complaint alleging something black being found in an order of chicken. The chicken was returned. The complainant alleges that the second order of chicken was brown and slimy. I conferred with assistant manager. She stated that the chicken is inspected closely. She stated that perhaps the darkness could have been due to the marrow close to the bone. A 5 gallon bucket is full of wastewater under xcel oven. I was told this has a leak but no one has dumped it. Repair this and keep bucket from overflowing. I was told this facility will be remodeled in mid August. Please provide the Health Department with plans for this renovation. I am issuing the permit to operate based on good faith that these items will be resolved with the remodeling. Policy for workers with painted fingernails is for them to wear gloves when dishing up food. This satisfies the regulations but must be enforced by manager. Worker did not put on gloves until I asked about company policy.

Observation: Observed working drinking from open cup, discard and proceeded to work the front line without washing her hands.
Correction: PIC intervened and had worker wash her hands.

Correction: Keep covers in place to prevent possible obstructions within lines.

Observation: Several areas of floor are in bad repair some of which include inside the middle walk in, threshold to chicken walk in, coving by prep sink/3-c sink, left of freezer walk in. Ceiling in middle walk in is pitted.
Correction: Floors and ceiling to be smooth and cleanable. Store is soon to be remodeled and this is to be corrected at that time.

Observation: The following areas were found dirty: water pooling by front handsink by window, dusty ceiling above front line, mildew on wall of 3-c area, mildew on walk in ceiling, wall behind and beside prep table/flat pan storage is soiled, caulking is soiled by back handsink, fan grills inside all walk ins are dirty/dusty, interior door to middle walk in has build up, floor corner by oil tank is dirty, floor around oil tanks is dirty, wall behind cookline has dirty coving.
Correction: Clean thoroughly. Perhaps the remodel will be a way to correct the above, if not, then these areas need major attention.
