    Correction: No juice bottles allowed in ice bins. Only store ice scoop in ice (items were removed).

    Observation: Found large deep pan of cooked beef in walk-in at 48 degrees. Product was more than 24 hours old.
    Correction: Cooling foods are to be 41 degrees or less. Please cool foods to 70 degrees in 2 hours and to 41 degrees in 4 more hours. (food products were voluntarily tossed).

    Observation: Found pans of chicken and beef in walk-in cooler had no date label on them.
    Correction: All containers of food held for 24 hours or more in cold storage are to have a date label. Please provide labels.

    Observation: Found wooden composite shelf worn out.
    Correction: Kitchen equipment to be in cleanable condition. Please replace/repair shelf to cleanable state.

    Correction: Cheese sauce must be discarded. This continues to be an on going issue and must be address.

    Observation: Observed an employee cutting raw meat then remove his gloves, rinsed his hands then put on clean gloves.
    Correction: Hands must be washed with soap for 20 seconds, then rinsed and dried before putting on gloves. Discussed with management and management discussed with employee.

    Observation: Observed employee rinsing hands in the food prep sink.
    Correction: Must use hand sink.

    Observation: Observed cheese sauce that was prepared the night before at 48 degrees in the walk-in cooler.
    Correction: Cheese sauce was discarded by management.

    Observation: Observed the cheese sauce and the carnita not date labeled. Both of these products are ready to eat and cooked and cooled.
    Correction: Carnita was date labeled and the cheese discarded.

The menu's consumer advisory is very confusing. Several items are marked as being offered as under cooked or raw but it does not state what it is. Next menu change this needs to be changed so that it is clear.

    Observation: Observed rust on the shelf under the two prep tables next to the walk-in cooler.
    Correction: Remove rust and maintain.
  • Equipment - Non-Food Contact Surfaces and Utensils
    Observation: Observed mold in the soda gun at the bar.
    Correction: Clean and sanitize.
  • Plumbing System Maintained in Good Repair
    Observation: Observed the plumbing for the spray nozzle leaking.
    Correction: Repair.
  • Physical Facilities in Good Repair
    Observation: Observed floor tiles loose, broken or grout low on the cook line, behind the soda machine in the wait station and the corner of the bar. Also, tiles broken and dirty around the two Auto Shaams in the wait station. The wall is buckling at the end of the bar and wall is loose behind the shelves in the kitchen. The wall behind the one hand sink is not sealed tight and is unpainted cement board. The walk-in cooler door and wall is torn exposing the insulation.
    Correction: Repair all areas.
  • Physical Facilities - Cleaning Frequency and Restrictions
    Observation: Observed grease on the wall next to the "Auxiliary Drain" at the end of the hood.
    Correction: Clean.

    Observation: Observed a live mature roach on the wall behind the Fajitas vegetables.
    Correction: Eliminate pest using approved methods.
