Spoke with the owner. When menus are reprinted, they will include a consumer advisory for smoked salmon. Until then, PIC will write a consumer advisory on the chalkboard. Permit issued.
  • Critical: Food - Consumption of Raw or Undercooked Animal Foods (repeated violation)
    Observation: There is no consumer advisory and disclosure statement for the smoked salmon on the menu.
    Correction: Provide a consumer advisory and idicate which foods may be served raw/undercooked.
  • Equipment - Cutting Surfaces (repeated violation)
    Observation: Cutting boards are heavily scratched.
    Correction: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.
  • Pests - Controlling Pests* (repeated violation)
    Observation: Observed several drain/fruit flies in the kitchen.
    Correction: Eliminate harborage conditions. Contact pest control company if needed.

    Observation: Handle of ice scoop is in contact with the ice. Utensils for espresso machine stored in a cup of water.
    Correction: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F.

    Correction: Provide a thermometer for each refrigerator.

    Observation: Cutting boards are heavily scratched.
    Correction: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.

    Observation: Observed food residue on the shelf below the slicer.
    Correction: Maintain nonfood-contact surfaces of equipment clean.
  • Handwashing Cleanser - Availability
    Observation: Soap was not provided at the hand washing lavatory in the kitchen.
    Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.

    Observation: Observed employee washing hands in the 3 compartment sink.
    Correction: Hands must be washed only in the hand washing sinks.
  • Food - Packaged and Unpackaged - Separation, Packaging, and Segregation
    Observation: Observed raw eggs stored directly on top of fresh strawberries.
    Correction: Do not store raw eggs with ready to eat foods.
  • Critical: Food - Consumption of Raw or Undercooked Animal Foods
    Observation: Observed cold smoke salmon in the refrigerator and listed on the menu without a consumer advisory. This is not a fully cooked product and consumer must be informed that there is increases their risk of foodborne illness.
    Correction: Provide consumer advisory.
  • Critical: Equipment - Food Contact Surfaces, Nonfood Contact Surfaces, and Utensils* (repeated violation)
    Observation: Observed dried food on the blade protector on the slicer.
    Correction: Clean and sanitize.
  • Critical: Handwashing Lavatory* (corrected on site)
    Observation: Observed service trays and a container of potato chip being stored in the hand sink in the processing area. Note, sink was filthy and had not been used in awhile.
    Correction: Keep sink open and usable. Corrected by EHS so that she could properly wash her hands.

    Observation: No soap at hand sink.
    Correction: Provide soap.

    Observation: Observed food build-up on the handle of the 3 door unit and the gaskets on the unit.
    Correction: Clean.
