- Observed ice behind the bar without a lid. Provide lid for ice when it is not in use.
- Observed the ice scoop stored on top of a towel above the ice machine. Store the ice scoop in a way that it is not exposed to the heater that is also stored on the ice machine, on an easily cleanable non absorbent surface.
- Observed the microwave and the freezer of the white Crosley cooling unit to have food debris build up. Clean and maintain routine cleaning.
- Observed the faucet in the women's restroom to have a leak. Repair.
- Observed the dumpster lid open. The dumpster should be kept closed at all times to detract pests/rodents.
- Observed the floors throughout the facility with missing or cracked tiles. Floors shall be in good repair and easily cleanable.
- Observed the walls and ceilings throughout back rooms of facility, and in both walk in coolers are made of materials that are not smooth, durable, or easily cleanable. Repair/replace.

- Observed ice machine door open. The manager stated that the bartender last night may have left the door open. Ensure the door to ice machine is closed when not in use to prevent possible contamination.
- Observed unmarked personal food items in walk in cooler and in refrigerator. Ensure all personal items are labeled.
- Observed buildup in microwave and in refrigerator. Clean and ensure routine cleaning.
- No self closing devices on either of the restroom doors. Provide self closers to rest room doors.
- There were no paper towels at the handsink in kitchen. Provide paper towels.
- Observed open dumpster lid. Please close lid to dumpster when not in use.
- Floors in facility have several missing or cracked tiles. Floor in back room made of exposed concrete. Floors shall be in good repair and easily cleanable.
- Walls and ceilings throughout back rooms of facility, and in both walk in coolers are made of materials that are not smooth, durable, or easily cleanable.
- Found no light shields on lights in back rooms. Provide light shields to cfl's.
- Observed chemicals stored above styrofoam cups. Cups were moved b staff at time of inspection.
