  • Found bottle of cooking oil unlabeled.
  • Container of cooked meat balls in lower reachin cooler were not labeled with date/time of cooldown.
  • Found ambient temp of front reachin cooler 47*. Found temp of lasagna & raviola 46* at 2PM. Food mgr said food temps were taken & were ok when they opened at 11AM. She moved above food to walkin cooler to cool down to 41*. She called repairman to repair. Do not store PH foods that require refr in this reachin cooler until fixed.
  • One employee did not have on a beard guard. Another employee with long hair was pinned up but not effectively restrained with hair net or hat, etc.
  • The door seal to lower reachin cooler & door seal to walkin cooler are badly worn/ broken/loose.
  • Found moist wiping cloth stored on counter.
  • Found lent/soil buildup on circ fan. Also found food/grease/scum buildup on some equipment/metal shelves/containers in kitchen.
  • The faucet for three comp sink is leaking.
  • The floor/wall molding is missing between reachin cooler & oven.
  • Ceiling tiles in kitchen are stained/dirty. Mgr said they have new ones & will install them soon.

  • The following were voluntarily removed from the facility: 8-2 lb packages of roast beef, 1 large container of roast beef, 1 large container of meatballs, 6 small containers of tuna salad, 1 small container of cookie dough, small amount of sausage.
  • The reach in cooler in the bread making area is ~50f. We made sure it was left closed and waited over 10 minutes to see if it would go down in temperature, but it didn't. The following were 46-48f, removed, and taken home by the mananger: 8-2 lb packages of roast beef, 1 large container of roast beef, 1 large container of meatballs, 6 small containers of tuna salad, 1 small container of cookie dough, small amount of sausage. The lasagna was in the process of being cooled down so it was moved to the freezer. Everything else in the cooler was moved to the other cooler. The manager called to get it fixed during the inspection. Before leaving I checked the cooler and it still read 50f.
  • Observed employees not wearing effective hair restraints.
  • There is tape on the seal of the walk in cooler door. The side of the counter of the hand sink has exposed wood. Many of the seals to reach in coolers/freezers are broken and damaged. The bottom of the cabinet under the soda fountain has what appears to be water damage.There's tape on the top of the soda fountain.
  • There is food buildup in handles,racks and shelving throughout facility. Observed by the walk in cooler a cart that had an accumulation of buildup.
  • The coving is missing on the walls on both sides of the register. On the left side there is a gap between the floor and wall. Coving also missing behind 3 compartment sink in kitchen.
  • Also the wall is damaged by the door and large stand mixer.
  • Chlorine sanitizer concentration was too high. When testing it the first time it turned the strip black and smelled strongly of bleach. The manager poured out most of it and put more water in a couple of times, but it was still black. She finally remade the sanitizer to be 200ppm. She told the employee who made the sanitizer how to make it properly.
  • There wasn't any temperature documentation at time of inspection. Per manager she took it home to make copies. Submit 2 weeks of temperature charts to me.

  • Observed an employee with bracelets on both wrists while working with ready to eat foods with no gloves. Explained that they can get dirty and hinder proper handwashing. The employee removed his bracelets. Due to having a bare hand policy it is even more vital that you only wear plain wedding bands and medical alert bracelets. Make sure you add this to your bare hand policy if it isn't there already.
  • Observed employees not wearing effective hair restraints.

  • Found only sanitizer bucket to have 50ppm of chlorine. Per manager it was made this morning.
  • There is food buildup in handles,racks and shelving throughout facility. Observed by the walk in cooler a cart that had an accumulation of buildup.

  • Found ceiling tiles in kitchen in disrepair. Per manager they have new ceiling tiles to replace the old ones. Also the wall is damaged by the door and large stand mixer.

  • I found towels/rags in multiple places in the facility including 2 in front of the large stand mixer, one on the 3-compartment sink, and in the dough/bread area. Towels should be stored in sanitizer or used then put in dirty linen area. You can not wipe your hands on towels.
  • There is food buildup in handles,racks and shelving throughout facility.
  • The coving is missing on the walls on both sides of the register. On the left side there is a gap between the floor and wall. I pointed this out to the manager.
  • Found ceiling tiles in kitchen in disrepair. Repair or replace. Also the wall is damaged by the door and large stand mixer.
  • There was a bottle of Hot Shot for flying insects stored with cleaners. The pesticide was voluntarily thrown away. Discussed that cleaners and pesticides must be stored away from cleaners. Also that flying insect spray only works when it makes contact with the insect, meanwhile you are spraying chemicals all over the facility.
  • The facility's most recent temp charts were from 2-6. Per manager she was away on vacation for 3 weeks. The employees must know that they are required to document temperatures, especially cool down. Submit 2 weeks of charts to me.
