  • Per manager she has done another half hour training and watched every employee properly wash their hands. The employee in the kitchen put soap on her hands arms for a couple of seconds, rinsed very little, then got a paper towel to touch the handle of the faucet. She still had soap all over her arms. I told her the whole process should take about 20 seconds and scrubbing should be about 15 seconds. She washed her hands again. Discussed with manager possibly having employees take the food handler class for extra training. Some employees may also need more training.
  • There are still over 5 flies in the kitchen. They were mainly under the 3-compartment sink. The facility has put up a couple of fly strips as well as have a fan to help. There were several dirty dishes and the prep surfaces had food as well. The donuts on the rolling cart were uncovered. Discussed making sure the kitchen is kept clean and foods left covered. There are also still mouse droppings in the back area. I pointed these out to the manager. An employee started cleaning at time of inspection. The manager will contact a licensed pest company today.

  • The metal stemmed thermometer used in the kitchen read 10f in an ice water bath. Showed employees how to calibrate thermometer.
  • In the kitchen there were several containers of icing that were left uncovered. The employee seemed to have gotten distracted and was doing other tasks. Also the donuts were left uncovered, but the employee covered them later into the inspection. It is vital that you keep foods covered while you have so many flies.
  • The employee in the kitchen went from cleaning to doing food prep without washing her hands. She didn't wash her hands the entire time I was in the kitchen. She seemed very flustered. I explained it to the manager that employees should be trained so that washing their hands is second nature even when they are flustered. You must conduct a training going over proper hand washing, when to wash hands, and the norovirus handout. I will check documentation of this when I conduct a reinspection on the pest issue.
  • There is shelving in the kitchen under hot holding and the cabinets under soda and condiments that have the coating coming off so there's wood exposed underneath. I pointed this out to the manager. Wood is not easily cleanable. The wood must be sealed or replaced with something easily cleanable such as stainless steel. Also the seal on the reach in cooler that has sandwiches in it is broken. There are labels coming off the side of the oven.
  • Found wiping clothes stored on counter. Store in sanitizer pail between uses.
  • The sides of the donut robot had an accumulation of grease.
  • In the utensil drawer there was buildup and food debris intermingled with clean utensils. Also the bottom of the reach in cooler for pizza there was food debris. There is dust in the fans of the walk in cooler.
  • The hand sink in the kitchen is coming away from the wall.
  • Observed almost 10 flies in the kitchen alone. They were landing on me, food contact surfaces (prep counter and mixer bowl), and food. Flies have the potential to contaminate food with Shigella. It is very important that you cover foods and clean often. Also saw what appeared to be a couple of mouse droppings in the room with the soda lines and under the soda fountain. It is a lot fewer then when I was here last. You need to address both of these issues. I will conduct a reinspection to check both.
  • The floor by the dough roller is damaged. I pointed it out to the manager. In the walk in cooler there were a couple of mop heads on the floor.
  • In the floor of the walk in cooler there were several cake bites.
  • There are many ceiling tiles that are stained and damaged. In the kitchen, room with the ice machine, and store area.
  • There is a light in the kitchen that isn't shielded.
  • The only food manager's license that is posted has expired early this year. The store manager was not available to ask if she has attended the class. You need to have someone licensed.

  • There is buildup in handles throughout the kitchen such as the prep table and the hot holding containers.
  • The handsink by the ice machine and both bathrooms didn't have hot water. I ran the water in all of them and they didn't change temperature at all. I went back later with the manager and she verified the water wasn't getting warm at all. During inspection she made calls to get this fixed. Once the plumbing issue has been fixed contact Katie so that I can verify. Until it is fixed make sure employees are washing their hands before bagging ice and in the restrooms.
  • In cabinets throughout the facility, such as under the soda fountain I found rodent droppings. I pointed them out to the manager. She said she didn't think they had a licensed pest control company treat the facility. Discussed cleaning then monitoring. Also putting down traps and not using anything with poison pellets. I will conduct a reinspection to check the status.
  • There are floor tiles in the kitchen that are damaged and coming away from the floor. The flooring is old and worn. The floor needs to be easily cleanable and in good condition.

  • Found cambro's of lunch meat, and protein salads without labeling. Ensure all ready to eat foods are labeled and follow the 7 day rule. Unlabeled foods were voluntarily disposed of by manager.
  • Found eggs stored above ready to eat foods. Ensure potentially hazardous foods are stored beneath ready to eat foods to avoid cross contamination. Eggs were moved by manager at time of inspection.
  • Found cardboard boxes used to store parchment paper and foil liners. Found cardboard used to line shelves in walk in. Found latch on top cooler of prep table in disrepair. Non-food contact surfaces shall be smooth, durable, nonabsorbent, and in good repair. Replace.
  • Found several moist towels stored on countertops. Store towels in sanitizer bucket.
  • The following items and surfaces were found with a accumulation of debris: Iced coffee machine, Microwave, Beneath soda machine, Ice machine, Soda racks in walk in cooler. Wash, rinse, and sanitize these items and surfaces and ensure routine cleaning.
  • Paper towel dispenser in back storage area near ice machine was out of paper towels. Ensure paper towels are continuously provided.
  • Found rubbing alcohol stored on shelf along side baking supplies. Toxic chemicals must be physically separate and below food and food contact surfaces. Alcohol was moved by manager at time of inspection.
