- Observed the ice scoop stored in a container that was wet and had wet paper in it. A different container for the ice scoop was provided during the inspection.
- Observed an open can of soda in the kitchen. Personal drinks should have a lid and straw and be kept separate from facility food. Soda was discarded during the inspection
- Observed an employee preparing food without an effective hair restraint during the inspection. A hat or hair net should be worn when handling food
- Observed a chlorine sanitizer bucket at 10-50 ppm. Chlorine sanitizer buckets/spray bottles should be 100-200 ppm. The sanitizer was corrected to 100-200 ppm during the inspection.
- Observed the inside of the ice machine to have black debris build up. Routinely wash, rinse, and sanitize the ice machine.
- Observed the handle to the reach in cooler sticky. The two standing fans in the kitchen, and the fans in the walk in coolers have dust accumulation. Clean and maintain routine cleaning.
- Observed the floor in the walk in coolers to be peeling. Per the manager the floors are going to be re painted in the summer.
- The wall behind the 3 compartment sink and the hand washing sink are peeling. Repair.
- Observed the wall fan near the 3 compartment sink with dust accumulation. Clean and maintain routine cleaning.

- There is no hot water in the restroom that is located in the side party room. Per the manager, it appeared that the hot water heater that serves that area of the building was not working. Provide hot water to handsink by 11/9/15.
- Observed floor in dough room that is not sealed. Seal. Observed coving in large walk in cooler that is becoming detached from the floor. Seal.
- Observed peeling paint on walls near 3 compartment sink, on walls in dough room, and on ceiling in the walk in cooler that is located in the front kitchen area. Seal or repaint.

- Observed cambros stored on shelves in a way that could allow dust and debris to settle in cambros. Store inverted.
- Staff at facility does not have food handler certifications. Per manager, a class was attended by all staff in March, 2015. They have not received their certificates. Ensure a copy of all food handler certificates are at facility.

- Observed uncovered items in white freezer. Observed uncovered butter in walk in refrigerator. Ensure all items are covered during storage.
- Observed the following items chipping or peeling: White shelf near dishwashing unit, and Window wall to dinning room. Repaint or reseal. Observed untreated wood shelving near walk in refrigerator. Seal or replace.
- Observed the following items/surfaces with an accumulation of debris: All carts in facility, white freezer near dish area, beverage cooler in drink storage area, Shelving units and racks throughout facility, and inside soda gun holders. Wash, rinse, and sanitize these surfaces/items and ensure routine cleaning.
- Observed trash can in employee bathroom with no lid. Provide lid.
- Observed rodent dropping on floor in kitchen. Clean floors. Discussed cleanliness and integrated pest management with manager. Manager stated he was aware of the problem and has has a licensed pest contractor out to the facility. I confirmed that a licensed pest contractor had been at the facility on 12/18/14. - Observed articles of clothing, personal items, and shoes stored on and around food items. Remove any personal items from food storage areas.
- Observed insecticide stored next to cleaning chemicals. Ensure these items are stored physically separate. Insecticide was removed by employee at time of inspection.
- Observed insecticides that were not food grade approved. Items were removed at time of inspection by manager.

- Found pre cooked sausage and sauce stored in the walk in with out labels. Only 2 day supply stored in cooler. If you only did one day storage it would not need label, since it is for two days it must be labeled.
- Found items (sliced onions, peppers, etc...)in the walk in cooler stored uncovered.
- Found straws and lemons in hand washing sink in kitchen. Corrected and spoke to employees.
- Non food contact surface cleaning: lime accumulation on the outside of the ice machine, flour accumulation on fixtures by dough area, clean the shelves in the walk in cooler, defrost beer glass cooler, wipe out keg cooler floor, clean as needed.
- The floors in the walk in coolers need to be repainted.
- The hood needs professionally cleaned. Excess of baking residue on the ceiling and hood. Inspector will follow up in 10 days.
