- Observed a Quat sanitizer bucket at 0-100 ppm. Am employee re filled the bucket, it tested at 200-400 ppm. Per the employee they were using the sanitizer bucket frequently during the lunch shift. During times of heavy use, sanitizer should be changed more frequently.
- Observed the ice dispenser on the soda machine with black debris build up. Clean and maintain routine cleaning.
- Observed dust accumulation on the monitor and wall behind the counter top bread slicer. Clean and maintain routine cleaning.

- Observed black growth buildup inside ice machine. Wash, rinse, and sanitize and ensure routine cleaning.
- Observed rodent droppings in cabinets beneath soda machine in dining room. Per the manager Janice, she has not seen any rodents at the facility. The facility does have a active pest management contract with Terminix. Terminix services this facility on a monthly basis. I reviewed the records and the facility was last treated 6/4/15. It was noted on the Terminix report that there was food debris and buildup in the cabinets beneath the ice machine. No evidence of pests were seen at that time. The cabinets were cleaned during the time of this inspection. The manager, Janice, called Terminix and explained the problem and Terminix said they will be out to treat the facility within the week. Left information regarding integrated pest management at facility.

- Observed employee wash hands and then turn off the faucet with bare hands. This re-contaminates hands. I did hand washing training with staff during inspection and had staff members properly wash hands.
- Observed gasket on small reach in cooler with gapped seal. Repair or replace.

- Found single service clam shells for sandwiches stored with food contact surface exposed. Corrected by inverting clam shells.
- The faucet at the prep sink has a small leak in it and will not turn off all the way. The hot water at the hand sink makes a very loud noise when in use. Concern is with vibrations causing connections to come loose.
- Found chlorox bottle filled with de-greaser. Bottle had both labels on it. Do not refill chemical spray bottles with anything other than the original chemical. This could cause a gas reaction. Corrected by speaking to manager and discarding spray bottle.
