Observation: Test strips are not available for checking sanitizer concentrations of the chlorine dishwasher or the quaternary ammonium used for sanitizer buckets.
Correction: Provide a test kit or other device for measuring the concentration of chlorine AND quaternary ammonium sanitizing solutions.

Observation: There is no Wisconsin Certified Food Manager for this establishment.
Correction: Provide a Wisconsin Certified Food Manager and post the state certificate in the food establishment within 90 days.

Observation: The right drawer bottom in the kitchen is delaminating.
Correction: Provide non-food contact surfaces that are constructed of a corrosion resistant, nonabsorbent, and smooth materials.

Observation: The refrigerator gaskets are in bad repair.
Correction: Repair equipment to good condition or remove from premise.
- NON-FOOD CONTACT SURFACES - CLEAN
Observation: Non-food contact surfaces of interior and exterior of the refrigerator and freezer need additional cleaning.
Correction: Clean and remove dust, dirt, food residue, and other debris from all non-food contact surfaces of equipment.
- UTENSILS, EQUIPMENT AND LINENS - STORING PROHIBITIONS - CLEAN DRY LOCATION
Observation: Boxes of single service cups are improperly stored on the floor.
Correction: Clean equipment and utensils, laundered linens, single-use and single service articles shall be stored in a clean dry location, not exposed to contamination and off the floor.

Observation: The wall behind the mop sink is chipping and is no longer easily cleanable.
Correction: Provide floors, walls, and ceilings that are designed, constructed, and installed so they are smooth and easily cleanable.
- TOILET FACILITIES - CLEANING OF PLUMBING FIXTURES
Observation: The urinals in the men's bathroom need additional cleaning.
Correction: Clean plumbing fixtures as often as necessary to keep them clean.
