Observation: Bag of bulk onions observed stored in a crate on the floor in the back storage area.
Correction: Store all food items 6 inches above the floor.

Observation: Water spray bottle near equipment storage observed with no labeling of bottle contents. Label added by manager at time of inspection.
Correction: Packaged foods shall be labeled as required by law.

Observation: Shelving unit above the 4 compartment sink observed with coating worn off. Cardboard observed being used to line dry storage shelving throughout the facility..
Correction: Provide non-food contact surfaces that are constructed of a corrosion resistant, nonabsorbent, and smooth materials.

Observation: The floor tiles are chipped in the back prep area. Small holes observed in wall paneling near the back 4 compartment sink.
Correction: Provide floors, walls, and ceilings that are designed, constructed, and installed so they are smooth and easily cleanable.

Observation: Foods in the sandwich bane were observed to be uncovered.
Correction: All foods that are not part of a cooling process shall be covered at all times to prevent cross-contamination. If cross-contamination is evident, discard suspected foods.

Correction: Chemicals used for disinfecting work surfaces must be of an approved sanitizing solution.

Observation: Food is not being properly cooked in Microwave. [chicken was at 80 F - 135 F initially]
Correction: Adjust methods or procedures to assure food heated in a microwave is covered, rotated and stirred midway through the cooking process, heated to 165°F and allowed to stand for two minutes after cooking.
