- Critical: PACKAGED AND UNPACKAGED FOOD - RAW OVER RTE-FOODS
Observation: There are ready-to-eat sprouts stored under raw chicken in the walk-in cooler.
Correction: Reorganize storage so that ready-to-eat foods are stored above raw animal food and raw vegetables. Discard ready-to-eat foods that show evidence of contamination.
- Critical: RTE/PHF-TCS PROPER DATE MARKING
Observation: Egg rolls and crab rangoons are not date marked
Correction: Adjust method or procedures so that all ready-to-eat potentially hazardous foods (time/temperature control for safety food), if required, are provided with date marking or an effective method of identifying when a food item has exceeded 7-days.
- FOOD CONTACT SURFACES - SOILED
Observation: Ice machine, soda gun, and meat slicer are visibly soiled. Clean utensils were also found to have food debris build up and were stored in a soiled bucket.
Correction: Maintain food contact surfaces in a clean condition.
- OPEN DRINK CONTAINER
Observation: Drinking cup without cover was observed in the two door true cooler.
Correction: Provide approved beverage container in food preparation area.
- IN-USE UTENSILS, BETWEEN USE STORAGE
Observation: Rice scoop is stored in a bucket of stagnant water. Scoops were improperly stored in bulk dry ingredient bins. Ice scoop is being stored in a bin containing empty water bottles
Correction: Store in-use utensils in the food item with handle extended, on a clean surface, running dipper well or in a container of water greater than 135°F.
- WIPING CLOTHS - USE LIMITATION - FOOD SPILLS
Observation: Wiping cloth solution at 0 ppm.
Correction: Wiping cloths soultion should be at 50 ppm
- WIPING CLOTHS - USE LIMITATION - CLOTHS FOR WIPING COUNTERS STORED IN SANITIZER (repeated violation)
Observation: Wiping cloth used for wiping counters stored on counters throughout restaurant.
Correction: Cloths used for wiping counters shall be stored in a sanitizing solution.
- FOOD CONTAMINATION PREVENTED FROM PREMISE - OFF THE FLOOR
Observation: Multiple food items are stored on the floor in the walk in cooler and freezer.
Correction: Store all food items 6 inches above the floor.
- FOOD CONTAMINATION PREVENTED - MISCELLANEOUS CONTAMINATION
Observation: Food container is being stored on top of an in use garbage can while prepping.
Correction: Change methods or procedures to protect foods from contamination.
- THAWING
Observation: Raw chicken is being improperly thawed by placing directly in two compartments of the four compartment sink.
Correction: Adjust procedures or methods to properly thaw foods.
- EQUIPMENT AND UTENSILS - MATERIAL FOR CONSTRUCTION AND REPAIR
Observation: Foods in walk-in freezer are stored in plastic grocery bags. Food in True 1 door cooler are being covered with fabric napkins and cardboard. Sliced peppers are stored in a bucket lined with a fabric napkin.
Correction: Discard equipment and utensils that are not: corrosion resistant, durable, able to withstand repeated warewashing, smooth, easily cleaning, nonabsorbent, and resistant to pitting, scratching, scoring or decomposition.
- FOOD PREPARATION SINK
Observation: Significant food preparation is occurring without the use of a food preparation sink and food preparation is occurring below the flood level rim of a sink compartment.
Correction: A food preparation sink shall be provided for significant food preparation or in instances where food preparation is occurring below the flood level rim of a sink compartment such as thawing or vegetable washing.
- UTENSILS, EQUIPMENT AND LINENS - PRESET TABLEWARE
Observation: The preset tableware is not properly covered to prevent contamination of food contact surface.
Correction: If preset, utensils shall be protected from contamination by being wrapped, covered or inverted. Any exposed, unused settings shall be removed when the consumer is seated or unused utensils shall be removed after the consumer is seated and be properly cleaned and sanitized.
- GARBAGE/REFUSE - OUTSIDE RECEPTACLES
Observation: The covers for the receptacles for dumpsters are open.
Correction: Provide tight fitting lids, doors and covers for the receptacles and waste handling units used for refuse, recyclables and returnables. Install receptables in a manner to prevent the accumulation of wastes and rodent harborage areas and locate receptacles so they can be easily cleaned and flushed with water.
- INSECTS AND RODENTS - OUTER OPENINGS PROTECTED
Observation: There is a hole or gap under the exit door off of the kitchen which could allow the entrance of insects or rodents.
Correction: Repair this hole or gap so that entrance by insects or rodents into the food establishment is prevented.
- CLEANING, FREQUENCY AND RESTRICTIONS (repeated violation)
Observation: The following areas need to be cleaned:
Food debris under cook line
Dust on ceiling, particularly the sprinkler and vents
Food debris on walk in cooler freezer and cooler floor
Food splatter on walls throughout kitchen.
Correction: Clean the physical facilities at a frequency necessary to keep them clean.
- PUBLIC RESTROOMS - GOOD REPAIR AND SUPPLIED WITH TOILET TISSUE
Observation: Women's restroom had signs of urine on side of toilet and floor.
Correction: Provide toilet tissue and maintain toilet in a clean condition.
