- Critical: DATE MARKING - DISPOSITION (corrected on site)
Observation: Hot dogs in package opened five days ago is not provided with a date mark.
Correction: Discard Ready-to-eat potentially hazardous (TCS) foods requiring date marking that have exceeded their discard date or are not date marked. Operator date marked hot dogs with opened date during inspection.
- Critical: TOXIC SUBSTANCES - SEPARATION (corrected on site)
Observation: Lysol air freshener and other toxic chemicals is stored next over the three compartment sink and drain board where clean utensils are stored.
Correction: Rearrange area so that toxic materials are stored away from food, equipment and single service items by spacing or partitioning. All items from the shelf above the three compartment sink were placed in the cabinet under the sinks during the inspection.
- EQUIPMENT AND UTENSILS - NONFOOD CONTACT SURFACES - CONSTRUCTED
Observation: Particleboard shelving under slushie syrups is not nonabsorbent and has soaked in spilled syrups.
Correction: Replace shelving and provide non-food contact surfaces that are constructed of a corrosion resistant, nonabsorbent, and smooth materials. Correct By: 04-Sep-2015
- INSECTS AND RODENTS - OUTER OPENINGS PROTECTED
Observation: (REPEAT) There is a gap at the bottom of the front entrance/exit doors which could allow the entrance of insects or rodents.
Correction: Repair this hole or gap so that entrance by insects or rodents into the food establishment is prevented. Recommended operator install additional flashing and/or brush trim Correct By: 18-Jun-2015
- CONTROLLING PESTS - INSPECTING PREMISE
Observation: Evidence of rodents and insects in and around building. There are wasp nests attached to the underside of the roof overhang, though no wasps observed. There is a significant amount of rodent feces throughout the facility, including in the drawer above single-use sandwich bags where they were found on the last routine inspection on August 13, 2013. There is torn up paper in the drawer as well which appears to have been chewed on by rodents. There are fire-starters in the corner of the store which appear to be distubed by rodents and surrounded by rodent feces. There are also rodent droppings along the walls, under low shelving, and in corners of the store and rear stockroom. The heaviest concentration of rodent droppings is next to the locked wood chest outside. No live or dead rodents observed.
Correction: Rountinely inspect facilities for insects and rodents. Remove all evidence of pest activity, inside and outside, within one week. Monitor for pest activity routinely (recommend daily) and adjust pest control measures to keep rodents out of the building and discourage them from congregating on the outside of the building. Recommended employees be retrained on posted closing procedures which include sweeping and mopping with a focus on monitoring for pest activity. Correct By: 11-Jun-2015
- CONTROLLING PESTS - HARBORAGE CONDITIONS
Observation: The locked wood chest for boating equipment on the exterior of the building has a concentrated amound of rodent droppings on both sides and is providing harborage conditions rodents. There are trapped leaves inbetween the rear fence and ice storage freezers. Used egg cartons with combustible material inside sold as fire starters are an attractant to rodents for their use as bedding. A rodent nest was found under the enclosed, hollow shelf used to store and flatten cardboard.
Correction: Store fire starters in sealed containers or wrapping to discourage rodents from using them as bedding. Clean out leaves and debris from the exterior of the building. Remove all unnecessary items from this area to minimize harborage for pests. Recommended wood chest be stored at least thirty feet away from the building or redesigned so that it does not rest on the ground. Correct By: 18-Jun-2015 Remove or redesign enclosed hollow shelving within 90 days.
