51 C (124 F) in Agra. I only spent 10 minutes outside, and was dehydrated and overheated the next moment. I was rushed to the hospital that day after suffering from a severe heat stroke.

113 in LA, sep 27 2012, although it must have been hotter since the thermometers of the national weather service stopped working once the mercury hit 113, but unofficial thermometers marked 120s, there is a pic on google of a bank off Wilshire Blvd showing a reading of 121F, and the car I was in read 122F, it could have been because of the Urban heat islands though.

40.6°C/105.1°F with a dew point of 8.3°C/47°F in Las Vegas on August 20th 2013.

Hottest temp I've experienced in the Niagara area was 36.7°C/98.1°F with a dew point of 25.4°C/77.7°F on July 21st 2011 (St. Catharines airport).

Highest dew point is hard to say. Most likely 28°C/82.4°F on August 7th 2001 (Niagara international airport), though I was still a kid.

104F/40C, Dayton OH in July 2012. The highest temperature I've ever experienced also happened to be the day I was outside with no shade from 10am-6pm, and the dew point reached 71.
