    Observation: White plastic shelving unit is not elevated off the floor. Cleaning hazard exists.
    Correction: Alter shelving unit to allow for easy floor cleaning. Install castors, remove bottom shelf, or replace unit.

    Observation: Dead lamps in women's restroom, over Turbo Chef oven, & over blender table.
    Correction: Replace dead lamps.

    Observation: Faucet at handwashing sink next to prep sink loose.
    Correction: Secure faucet to handsink.

Purpose of site visit: Evaluate facility for foodservice permit.
Correct the following items immediately:
1) Repair rear kitchen handsink faucets.
2) Repair 3-vat sink drain line leak.
3) Install castors or approved 6" legs on plastic shelves in rear storage / office area.
4) Add 5th shelf to 18"X 72" wire shelving unit.
5) Replace badly corroded prep sink faucet.
6) Replace dead ceiling light lamps in front kitchen and in dining area.
7) Provide a minimum of 20 foot-candles of light in both restrooms. Measured 12-15 foot-candles.
8) Provide evidence of employee health policy (EHP) training.
9) Store disposable flatware with handles up or use prewrapped utensils.
10) Post "No Smoking" signage in dining area and/or at front entrance.
11) Do not store food on floor in walk-in freezer. Recommend additional 5th shelves in both walk-in units.
12) Store foam cups in original plastic sleeve or in an approved dispenser.
13) Clean ceiling vent grills in office/ storage area - dust.
14) Remove plastic milk jug crates from premises.
15) Store the ice bucket inverted to drain. Recommend a tray or sheet pan under bucket.
16) Do not store cutting board at 3-vat sink faucet.
17) Label plastic gallon jug with pump at front prep cooler.
18) Label large mobile bulb food storage container next to rear kitchen prep table.
Discussed EHP, handwashing, gloves use, no smoking, and critical items handouts with PIC.
Recommend an approved wall shelf above the kitchen window.
