Fire on grill. No suppression system disbursed. Employees put fire out with extinguishers. Fire approx. 10:30 am. Fire contained to cooking area. EHS observed raw chicken with debris. PIC discarded. All Food contact surfaces have been washed rinsed and sanitized. All counters and exterior of equipment have been cleaned. All floors, walls, hoods cleaned. Lunch dispensed in area of kitchen not contaminated by fire. Lunch was in warming units and protected. Facility cleared to re-open at this time.

    Observation: Observed several nicks on the meat slicer blade.
    Correction: Replace the blade to provide a food contact surface that is smooth and easily cleanable, free of breaks, open seams, cracks, chips, pits, sharp internal angles, corners, and crevices to prevent build-up of food debris.

    Observation: The atmospheric vacuum breaker on the threaded faucet at the powerwashing area is not an appropriate backflow prevention device for a threaded faucet to which a "Y" and 2 hose lines under constant pressure are attached..
    Correction: Provide proper backflow or backsiphonage prevention device designed for continuous pressure situations on the threaded faucet.

    Observation: Second sink basin at the 3-compartment sink observed leaking at seal on the drain release.
    Correction: Repair and maintain all plumbing components and fixtures.

    - interior bottom of the cook's drawer
    - metal shelf beneath the stainless steel prep. table at the salad prep. area
    Correction: Clean the surfaces at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

    Observation: Ceramic tile observed missing from the floor/wall juncture inside the Pull Box/walk-in refrigeration unit.
    Correction: Repair base of wall. Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
