The purpose of today's visit was to conduct a routine assessment. The following suggestions have been made: 1. Please review Employe Health Policy documents provided today (English) with all staff immediately. 2. Have multiple employees obtain Certified Food Manager (CFM) card. Make copies of CFM cards to keep on file in the event an employee forgets to bring theirs into food establishment. As a reminder, a CFM must be present in facility during all hours of food handling Please send the following to the Health Department within 7 days: 1. Copy of CFM card
- Duties / Inform Employees about Responsibility to Report GI Symptoms, Disease, Medical Condition or Exposure to PIC (corrected on site)
Observation: Employees are not aware of the reporting procedures concerning information about their health and activities as they relate to diseases that are transmissible through food.
Correction: The Person in Charge or certified food manager shall inform all food employees and conditional employees of their responsibility to report any information about their health and activities as they relate to diseases that are transmissable through food as specified under 2-201.11(A). PROVIDED EMPLOYEE HEALTH DOCUMENTS FOR EMPLOYEE TRAINING (ENGLISH).

Observation: There is no Certified Food Manager (CFM) present at the beginning of the inspection.
Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment. A CFM FROM A PARTNER FACILITY WAS ABLE TO ARRIVE BEFORE THE COMPLETION OF TODAY'S ASSESSMENT.

Recommendations made to make copies of all CFM cards and keep on file on site in the event that an employee forgets to bring in physical copy. Also made recommendation to put small handwashing durable placard on splash guard of handsink since business regulations prohibit hanging signs on wall above handsink. A laminated/metal placard on splash guard will be able to better withstand splashing of water by handsink than paper handwashing sign provided by EHS.

Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc. PROVIDED EXPIRED CFM CARD, OTHER EMPLOYEE FORGOT CFM CARD IN CAR.
Correction: Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448. Failure to provide a copy of the photo identification card may result in further enforcement action.
- Handwashing Signage at Handwashing Facilities Required (corrected on site)
Observation: Observed that sign or poster that notifies food employees to wash their hands is not provided at HAND SINK NEAR 3-VAT SINK used by food employees
Correction: Provide a sign or poster at all handwashing sinks used by food employees that reminds food employees to wash their hands. EHS PROVIDED HANDWASHING SIGN.

**Approval is hereby granted for the issuance of a Health Department Permit. This inspection report shall serve as your permit until the official permit is received. All outstanding violations are to be corrected within ten (10) days. Equipment additions/replacement/changes are not allowed without Health Department approval.** Note the following recommendations: It is recommended that more than one person on staff be a Certified Food Manager. A Certified Food Manager must be on the premises during ALL food preparation and service. It is recommended that a monthly pest control program be implemented. Maintain records of pest control treatments for Health Department review.
