Inspection for permit renewal.

Observation: Observed accumulations of encrusted grease deposits or other soil on the food contact surfaces of the stove and grill.
Correction: Clean and sanitize these surfaces for food contact.
- Light Bulbs Protective Shielding (repeated violation)
Observation: Light bulbs in prep area are not shielded, coated, or otherwise shatter-resistant.
Correction: Shield or replace light bulb with a coated or shatter-resistant bulb.
- Physical Facilities - Cleaning Frequency and Restrictions
Observation: Floors in kitchen noted in need of cleaning.
Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

Observation: Observed methods are not being used to control pests. Insects were present on the dish machine, in refrigerator, on floors, and in restrooms.
Correction: The presence of insects, rodents, and other pests shall be controlled to minimize their presence on the premises by using methods such as trapping devices or other methods as required. Person in charge said they will contact pest control company.

Observation: The following foods were observed cold holding at improper temperatures: chipotle dressing, peppers, corned beef hash, macaroni & cheese, milk, whipping cream, ranch dressing, butter, corn
Correction: No cooling unit in facility was able to maintain proper cold holding temperatures. Person in charge contacted refrigeration maintenance to repair all coolers. PIC then relocated all TCS foods to containers full of ice in order to maintain food storage at 41°F or below until the coolers were repaired.

Observation: Mechanically vented hood filters not in good repair or have gaps in between filters.
Correction: Repair or replace hood filters and/or eliminate gaps between filters.
- Equipment - Cutting Surfaces
Observation: The cutting board is heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
Correction: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.
- Equipment - Non-Food Contact Surfaces and Utensils
Observation: Surfaces of the ice machine was in contact with non-potentially hazardous food items that were observed soiled with accumulations of grime and debris.
Correction: Clean the surface of the ice machine at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications.

Observation: The food contact equipment surface of the ovens are observed soiled with accumulations of grime and debris.
Correction: Clean the food contact surface of the ovens to prevent accumulations that may reduce heat transfer to adequately cook food and to prevent attract of insects when not in use.

Observation: The nonfood contact surface of the storage shelf and the bottom compartment of the fryer has accumulations of grime and debris.
Observed accumulation of grime on the can opener.
Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

Observation: Single service items observed unprotected from contamination (straws and stirrers).
Correction: Store single service items in its original protective packaging or inverted in an approved dispenser.

Observation: Trash and litter were observed adjacent to the refuse container outside the facility.
Correction: The refuse container storage area and grounds adjacent to the container are to be maintained clean and sanitary. Facility is using a shared dumpster area. The PIC will have dialogue with building manager (s) to ensure all personnel using the dumpster will put all refuse inside the container.

Correction: Repair the floor and wall to ensure they are smooth, impervious and easily cleanable Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
