This visit was made to conduct a change of ownership inspection. Food establishment installed the following new hot water heater: State Industries (CSB12036SFEX 100) Input 45 KW *New hot water heater is sufficient to meet the required recovery rate for the entire grocery store. The following item shall be corrected within 30 days: 1. The new light shields are not adequately sized (appears to be too short) and shall be resized to be tight fitting. 2. Walk-in refrigerator sliding door is not tight fitting when shut and shall be repaired. 3. Rusted shelves shall be replaced throughtout (shelves on order) within 30 days. Note: Educational signs for the produce shink will be brought at your 30 day follow up inspection. *Approved for change of ownership, recommend issuance of Alexandria Health Department permit. This visit was made to conduct a change of ownership inspection. Prior to merge (re-opening), the following items require correction: 1. Hot water heat is 57% and 46% insufficient and shall be replaced with a hot water heater with enough input to meet facilities peak hot water demand. 2. Provide a mop rack at mop sink 3. Food prep sink is in poor repair (legs warped, rusted surfaces, and damaged cutting board) and shall be replaced. Additionally the food prep sink is not currently plumbed with a 1" air gap. After replacing prep sink make sure waste line has a 1" indirect drainage. 4. Wall (FRP board) behind the food prep sink is in poor repair (rusted, warped, and peeling from wall) and shall be replaced. 5. Floor coving behind food prep sink is peeling from the FRP board and shall be replaced/repaired. 6. Waste line for the water softener is currently on the floor and shall be installed/mounted up off the floor with a proper 1" indirect drainage. 7. Condenser waste line for the produce walk-in cooler needs to be mounted onto wall for adequate drainage into floor sink. 8. Provide a drain plug for food prep sink. 9. Garbel unit at food prep sink is in poor repair and no longer functional and shall be removed. 10. Rusted shelving throughout grocery is no longer cleanable and shall be resurfaced or replaced. 11. Cracked concrete in walk-in coolers shall be sealed to provide a surface that is smooth and easily cleanable. 12. Repair damaged thermometer for display refrigerator. 13. Lights fixtures for display refrigerators shall be properly protected with shields and end caps. 14. Damaged ceiling above walk-in refrigerators/freezers shall be repaired. 15. Damage door gasket for dairy refrigerator shall be replaced. 16. Remove wood shelving/pallets in walk-in refrigerators.
