This visit was made to conduct a change of ownership inspection. 3-compartment sink across cookline was removed to meet the requirement recovery rate of the new hot water heater. Specifications for new hot water heater: State Industries (CSB12036SFEX 100) Input 45 KW The following item shall be corrected within 30 days: 1. Dirty dish drainboard shall be resurfaced to provide a smooth and easily cleanable surface, along with adequate space to store dirty dishes. *Approved for change of ownership, recommend issuance of Alexandria Health Department permit. Note: Thank you for all the hard work all the departments made to make this transition a smooth process in regards to Alexandria Health Department requirements. This visit was made to conduct a change of ownership inspection. Prior to merge (re-opening), the following items require correction: 1. T-grid shelving rusted and shall be replaced. 2. Ceiling tiles in the dry storage room are no longer easily cleanable and shall be replaced. 3. The prep refrigerator at deli line is no longer working and shall be removed and replaced. 4. The 2-door undercounter refrigerator is no longer working and shall be removed. 5. Walk-in freezer floor tiles at juncture/walk-through are damaged and shall be replaced/repaired. 6. Shelving throughout department are no longer smooth and easily cleanable (rusted) and shall be replaced. 7. Burnt out light fixtures shall be replaced to provided sufficient lighting and all light fixtures shall be properly protected with covers or shields with end caps. 8. Flooring is in need of deep cleaning. 9. Coving at soda fountain machine is peeling from wall and shall be repaired/replaced. 10. Floor faucet is leaking water and shall be repaired. If faucet is no longer in use, cap to prevent long term maintenance. 11. Storage room with personal belongings shall be repaired/painted (chipped walls). 12. Hood no longer used shall be removed and the walls and ceilings shall be constructed of materials which are smooth, durable and easily cleanable. 13. Floor sink for 3-compartment sink is slow to drain and shall be maintained. 14. Clean floor in soda fountain cabinet. Note: Hot water heater is not sufficient in size and shall be replaced.
