The purpose of today's visit was to conduct a risk factor inspection. The following suggestions have been made as a means to incorporate Active Managerial Control:
1. Please have multiple Certified Food Managers (CFMs) available during a shift in the event that one needs to leave the establishment during hours of food handling. Suggestions made to make copies of CFM cards to keep on file in-house for quick reference.
2. Please train ALL employees on HACCP plan for Reduced Oxygen Packaging (ROP) prior to assigning them to vacuum seal products. Review HACCP regularly and reference it during ROP procedures.

  • Critical: Certified Food Manager/Presence Required (corrected on site)
    Observation: There is no Certified Food Manager (CFM) present at the beginning of the inspection.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment. CFM RETURNED FROM RUNNING ERRANDS PRIOR TO COMPLETION OF INSPECTION.

The purpose of today's visit was to conduct a routine assessment. Suggestions made to place grilled chicken sandwiches in walk-in freezer, rather than 1 door cooler at prep line, immediately after preparing to ensure items are cooled to 70F or below in 2 hours AND 41F or below within the next 4 hours.
  • Physical Facilities Good Repair (corrected on site)
    Observation: Observed that the CEILING ABOVE DRY STORAGE is not maintained in good repair.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities. REPLACED TWO MISSING CEILING TILES FOR CEILING LOCATED ABOVE DRY STORAGE AREA

1. The self-serve soda dispensing machine actuating lever or mechanism and filling device is not designed to prevent contact with the lip-contact surfaces of cups that atre refilled. Long levers or push buttons are required.
2. The mechanical ware washing machine is not operational.
*Owner / Contractor shall correct the above listed items and schedule for final follow up Health Department inspection.
*You may stock pre-packaged food items only. No food preparation or food service.
