The purpose of today's visit was to conduct a risk-factor assessment. The following violations have been observed as repeated for the second time:
1. Hand cleanser not present at each hand sink
2. Improper hand washing procedure. Hand washing training has been provided for the second time by the Health Department.
The following suggestions have been made:
1. Repair or replace hand cleanser dispenser so it is firmly fixed and mounted to wall for easy access
2. Review Employee Health Policy (EHP) regularly (biannually) for all long-term employees. Provide EHP training immediately and prior to food handling for all newly hired employees
3. Review "How to Prevent Cross Contamination" document with all food handling employees. English and Spanish Provided.
  • Critical: Hands and Arms / Cleaning Procedure (corrected on site) (repeated violation)
    Observation: Observed food employees using improper handwashing procedures. OBSERVED EMPLOYEE WASH HANDS FOR APPROXIMATELY 5-10 SECONDS WITHOUT USING ANY SOAP.
    Correction: Food employees shall wash their hands and exposed portions of their arms by: 1) discarding used gloves
  • Critical: Food Protection from Contamination / Separate Raw from RTE (corrected on site)
    Observation: Raw animal food holding in a manner that may cause cross contamination of ready-to-eat food as follows: OBSERVED RAW SHELL EGGS BEING STORED OVER RINSED READ-TO-EAT LETTUCE IN WALK-IN COOLER.
    Correction: Food shall be protected from cross contamination by separating raw animal foods like poultry, beef, pork, seafood, and eggs during holding from ALL other foods such as cooked foods, ready-to-eat foods, washed produce, etc. EGGS RELOCATED UNDER LETTUCE.
  • Handwashing Cleanser, Available at Handwashing Facilities (corrected on site) (repeated violation)
    Observation: Observed that hand soap was not provided at handwashing sink NEAR WALK-IN COOLER.
    Correction: Provide hand soap at each hand sink to allow employees to properly wash their hands. HAND SOAP PROVIDED.

The purpose of today's visit was to conduct a routine assessment. Please obtain CFM card with photo within 10 days. Copy of CFM card can be sent via fax or e-mail. Proper hand washing procedures are extremely important in ensuring food safety. Emphasized the need to make sure employees now how to wash hands correctly and when to wash hands to prevent the spread of food-borne illness. Provided detailed hand washing training to employees. Suggestions made to regularly train employees on proper hand washing (perhaps quarterly until appropriate hand washing techniques are observed). Also, active managerial control (AMC) discussed. Advised the implementation of food temperature logs, equipment temperature logs, cleaning schedules, etc.

    Observation: Observed food employees using improper handwashing procedures. OBSERVED THE FOLLOWING: 1) EMPLOYEE WASH HANDS FOR ABOUT 5 SECONDS WITHOUT USING ANY SOAP, 2) EMPLOYEE HANDLE DIRTY DISHES IN 3-VAT SINK AND THEN WRAP CUCUMBERS WITHOUT CHANGING GLOVES AND WASHING HANDS, 3) EMPLOYEE SWITCH TASKS AND DON NEW GLOVES WITHOUT FIRST WASHING HANDS
    Correction: Food employees shall wash their hands and exposed portions of their arms by: 1) discarding used gloves
  • Critical: Certified Food Manager/Presence Required (corrected on site)
    Observation: There is no Certified Food Manager present at the beginning of the inspection. CFM OUT OF STORE ASSISTING A DELIVERY AT THE BEGINING OF INSPECTION.
    Correction: It shall be unlawful to operate a food establishment unless it is under the immediate control of a certified food manager. It is highly suggested that you have more than one employee with the food manager's license. ORS Interactive, Inc. (see handout provided) issues the required photo identification card with proof of successful completion of a certified food manager's exam. Failure to have a certified food manager on site during ALL hours of operation including food preparation, food service, and cleaning/sanitizing of equipment/utensils will result in closure of the establishment. CFM WAS MADE AVAILABLE 5 MINUTES INTO INSPECTION.
  • Certified Food Manager/Certificate Process
    Observation: The person in charge (PIC) has a valid certificate of successful completion of a Certified Food Manager (CFM) exam but does not possess the CFM card issued by ORS Interactive, Inc.
    Correction: PLEASE OBTAIN CFM CARD FOR ALL EMPLOYEES WHO ARE SCHEDULED TO TAKE CFM EXAM. Food managers who possess a valid certificate of passing a food safety exam shall obtain the photo identification card from ORS Interactive, Inc. (see handout provided). A copy of the photo identification card can be faxed to the food safety section at (703) 653-9448.

Complaint states that smoking is occurring inside as well as outside the establishment. Environmental Health Specialist (EHS) discussed complaint with manager upon arrival. No evidence of smoking observed inside the establishment. EHS reminded manager that the establishment is non-smoking and asked that he remind his staff. A No Smoking sign is posed behind cash registers.
