Discussed employee health with the person in charge. The facility had great cold, cooking, and hot holding temperatures.

    Observation: Observed raw chicken over raw goat. Different types of raw animal foods stored in such a manner that may cause cross contamination.
    Correction: Separate types of raw animal foods from each other such as beef, fish, lamb pork, and poultry during storage, preparation, holding, and display by the following: (a) Using separate equipment for each type. (b) Arranging each type of food in equipment so that cross contamination of one type with another is prevented. (c) Preparing each type of food at different times or in separate areas.
  • Food Storage - Clean and Dry Location (repeated violation)
    Observation: Observed foods in the walk in cooler without lids or plastic wrap. Also the raw goat was sitting on the floor of the walk in cooler. Food stored in a location where it is subject to splash, dust or other contamination. Please use lids or plastic wrap to protect foods in the walk in cooler.
    Correction: Store food where it is not exposed to splash, dust, or other contamination to prevent contamination.

    Observation: Observed prepared ready-to-eat (RTE) foods in the refrigeration unit were not properly dated for disposition.
    Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.

    Observation: The nonfood contact surface of the floors, behind the dishwasher, the ice machine, and the ice guard inside the machine has accumulations of grime and debris.
    Correction: Please clean these areas at a frequency necessary to preclude accumulation of grime and debris.

    Observation: The ceilings in the facility are not smooth and easily cleanable. Please clean the ceillings.
    Correction: Repair or replace ceiling to make it smooth and easily cleanable.

    Observation: Observed the back door has a gap that could allow insects to come into the facility. Outer opening of the food establishment is not protected against entry of insects and rodents.
    Correction: Protect the food establishment against the entry of insects and rodents by 1. Filling or closing holes and other gaps along floors, walls, and ceilings, 2. Closed, tight fitting windows, and 3. Solid, self-closing, tight-fitting doors. Insects and rodents are vectors of disease-causing microorganisms which may be transmitted to humans by contamination of food and food-contact surfaces.

    Observation: Raw foods of animal origin stored next to ready-to-eat (RTE). Raw lamb and shrimp stored next to sour cream.
    Correction: Separate raw foods of animal origin during storage, preparation, holding, and display from raw RTE food including foods such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.

    Observation: The methods used for cooling were not adequate.
    Correction: Cool foods by the following methods: 1. Placing food in shallow pans 2. Separating food into smaller thinner portions 3. Using rapid chill cooling equipment 4. Stirring the food in a container placed in an ice water bath 5. Using containers that facilitate heat transfer 6. Adding ice as an ingredient 7. Any other method that can effectively accomplish cooling.

    Correction: Rapidly reheat the food to 165°F and maintain at 135°F or above through the hot holding period.

    Correction: Provide written procedures for the use of time as a public health control. The procedures must include the following steps or practices: a. The food is served or discarded within four hours from the time when the food is removed from temperature control c. Food in unmarked containers or packages, or for which the time expires is discarded and d. Written procedures are maintained in the food establishment and made available to the regulatory authority upon request e. Once time is implemented as a control measure for potentially hazardous food, no other measures may be substituted.

    Observation: Chlorine sanitizing solution used was not at an acceptable concentration. Chlorine was at acceptable levels the sanitzer was dispensed before the wash and rince cycle.
    Correction: Adjust the chlorine sanitizing solution to a level of at least 50 parts per million when the pH is at 8 or less and the water temperature is 75°F. Wash rinse and sanitize. Manager agreed to use three compartment sink until dishwaser cycles corrected.

    Observation: The nonfood contact surface of the reach in chest freezer has accumulations of condensate .
    Correction: Clean the surface at a frequency necessary to preclude accumulation of condensate.

    Observation: The cutting board(s) along the prep units are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.

    Observation: Surfaces of the ice machine was in contact with non-potentially hazardous food items that were observed soiled with accumulations of grime and debris.
    Correction: Clean the surface of @EQUIPMENT@ at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications.
