This visit was made to conduct a routine inspection. The following items require attention:
1) Store retail food items and food contact equipment separate from employee food items and dishes.
2) Implement an employee health policy to ensure that employees know reportable symptoms and illnesses, and managers know how to properly exclude and/or restrict employees..
  • Critical: Employee Health Policy Requirements (repeated violation)
    Observation: Through conversation and/or review of written policy, it was determined that the establishment's employee health policy (EHP) is not compliant with the Food Code because: (Manager could not tell big 5 illnesses and symptoms)
    Correction: Create and implement an EHP that: 1) requires employees to report certain symptoms (vomiting, diarrhea, jaundice, sore throat with fever, or a lesion/infected wound) and illnesses (Norovirus, Hepatitis A, Salmonella Typhi, E. coli, or Shigella)
  • Miscellaneous Sources of Contamination / Other Sources
    Observation: The following food item(s) were found subject to contamination after receipt and during storage due to the condition as described: Retail food items subject to contamination from employee food items stored above and next to them in the 1 door fridge in the kitchen.
    Correction: Food shall be protected from contamination that may result from a factor or source not specified under Subparts 3-301 through 3-306. (Store employee food in a separate, designated area)

Two commercial refrigerator units are now for personal used. The establishment had reduced the menu to canned Tuna and canned chicken to make sandwiches.

    Observation: Through conversation and/or review of written policy, it was determined that the establishment's employee health policy (EHP) is not compliant with the Food Code because: The CFM has no knowledge of the policy
    Correction: Create and implement an EHP that: 1) requires employees to report certain symptoms (vomiting, diarrhea, jaundice, sore throat with fever, or a lesion/infected wound) and illnesses (Norovirus, Hepatitis A, Salmonella Typhi, E. coli, or Shigella)
  • Critical: PIC Shall Exclude Food Employee with Vomiting or Diarrhea
    Observation: The CFM has no knowledge of the policy
    Correction: Handed hand outs to the CFM.
  • Maintaining Premises Free of Unnecessary Items and Unused Equipment (repeated violation)
    Observation: Observed that items that are unnecessary to the operation or maintenance of the establishment are being stored on the premises.
    Correction: Remove unnecessary items, including equipment which is no longer in use, from the premises to allow for regular and effective cleaning and to prevent harborage conditions.
  • Mops in Air-dry Position (repeated violation)
    Observation: Mops and brooms are not hung up to air dry.
    Correction: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.
  • Personal Care Items Stored in Employee Locker or Designated Area
    Observation: Observed personal items and merchandise stored in such a way that they could contaminate equipment in the establishment.
    Correction: All employees' personal care items shall be stored in a way that prevents the contamination of food and food contact surfaces through improper labeling or storage.
