This visit was made to conduct a routine inspection. Please note the following items:
1) Replace damaged ceiling tiles in the dry storage area and repair broken gasket on milk cooler.
2) Provide a thermometer for the walk-in freezer that is accurate to +/-3F.
3) Great hand washing observed by staff.
4) Good holding temperatures observed.

  • Food Storage / Preventing Contamination (corrected on site)
    Observation: Food stored on the floor and/or food stored less than 6 inches off the floor. Several boxes of food stored on floor in walk-in freezer.
    Correction: Food shall be protected from contamination by storing the food at least 6 inches off the floor on approved shelving units or dunnage racks. Milk crates, soda crates, or bread racks are not suitable for food storage.
  • Ambient Air & Water Temp Meas Devices, F Scale, Accurate within 3°F
    Observation: The ambient air temperature measuring device located in the following unit is not accurate in °F: Walk-in freezer (unit on exterior was reading 68F, and internal thermometer was reading 40F.
    Correction: Ambient air and water temperature measuring devices that are scaled only in Fahrenheit shall be accurate to +/-3°F in the intended range of use.
  • Equipment / Good Repair / Components / Gaskets
    Observation: The door gaskets of the following unit(s) are (missing, damaged): milk cooler #9263.
    Correction: Equipment components such as doors, seals, hinges, fasteners, and kick plates shall be kept intact, tight, and adjusted in accordance with manufacturer's specifications.
  • Physical Facilities Good Repair (repeated violation)
    Observation: Observed that the ceiling in the dry storage area is not maintained in good repair. Ceiling tiles are stained and have water damage.
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.

This visit was made to conduct a routine food safety evaluation. The person in charge was very knowledgeable and helpful. Great job on temperature control and keeping logs of equipment temperature.

  • Lighting Intensity/ Walk-ins & Dry Storage/ 10 foot candles
    Observation: Observed that inadequate lighting was provided in the walk-in refrigerators, walk-in freezers, and/or dry storage areas. (There are light bulbs out in the walk-in refrigerator and freezer)
    Correction: Increase the lighting to provide a minimum of 10 foot candles at a distance of 30 inches off of the floor in these areas.
  • Physical Facilities Good Repair
    Observation: Observed that the wall tiles throughout the whole kitchen are not maintained in good repair. (The wall tiles are cracked throughout, but particularly under the window and above the prep table next to the 3 comp sink)
    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.
  • Maintaining Premises Free of Unnecessary Items and Unused Equipment
    Observation: Observed that items that are unnecessary to the operation or maintenance of the establishment are being stored on the premises. (There is a 2 door reach-in fridge that is not functioning and has not been repaired for several months)
    Correction: Remove unnecessary items, including equipment which is no longer in use, from the premises to allow for regular and effective cleaning and to prevent harborage conditions.

Repair milk cooler as soon as possible since it is necessary for service.

  • Equipment / Good Repair / Operation
    Observation: The following mechanical refrigeration unit is not currently operating as required to cold hold food at a temperature of 41°F or less: Milk cooler in the center of the serving line near the kitchen entrance. (This unit is not functioning to maintain milk at 41 degrees F or less. Manager reported that they are putting milk out for service and then putting in the freezer to cool back down between services)
    Correction: Refrigerators shall be capable of holding foods at a temperature of 41°F or below. Repair the unit(s) immediately. Foods exposed to temperature abuse for over 2 hours shall be discarded, otherwise, the foods shall be stored in properly operating refrigerators. (PIC agreed to put ice or something frozen in the cooler to keep milk cold while it's out. He also agreed to put only as much milk as they need for one service out at a time. This unit is necessary for service, and needs to be repaired as soon as possible.)
  • Plumbing / Maintained in Good Repair
    Observation: The hand sink outside of the employee restroom has a contant stream of warm water, but the hot faucet does not function to provide enough hot water to wash hands. Cold water has to be used as well to have enough pressure.
    Correction: A plumbing system shall be maintained in good repair. Repair hot water faucet as soon as possible.

    Observation: Through conversation and/or review of written policy, it was determined that the establishment's employee health policy (EHP) is not compliant with the Food Code because: employee did not know BIG 5 illnesses or reportable symptoms. New employee health process tree and and poster reviewed with manager.
    Correction: Create and implement an EHP that: 1) requires employees to report certain symptoms (vomiting, diarrhea, jaundice, sore throat with fever, or a lesion/infected wound) and illnesses (Norovirus, Hepatitis A, Salmonella Typhi, E. coli, or Shigella)

    Observation: The following food item(s) were measured at improper temperatures: milk (in milk box on the left facing kitchen) all milk in this box was moved to colder unit.

    1. wall paint, tiles, and drywall are damaged throughout the kitchen, seving area, and bathroom. There is a hole where the garbage disposal was removed.
    2. Ceiling is damaged throughout. Ceiling tiles have evidence of water damage, and there is a hole in the drywall ceiling in the serving area.
    3. There is also water damage and chipping paint on the window sills in the kitchen.

    Correction: Repair the physical facilities. Poor repair and maintenance compromises the functionality of the physical facilities.
