    Observation: The diameter of the air gap between the water supply inlet and the flood level rim of the 3 vat sink is less than twice the diameter of the water supply inlet. Spray hose hanging in sink.
    Correction: Provide an air gap with a diameter that is at least twice the diameter of the water supply inlet and not less that 1 inch for proper backflow prevention by the air gap.
  • Lighting, Intensity
    Observation: Less than 10 foot candles of light was noted in the storage area. Multiple light fixtures not working.
    Correction: Provide at least 10 foot candles at a distance of 30 inches from the floor in the walk-in refrigeration units and dry storage areas and in other areas and rooms during periods of cleaning.
  • Physical Facilities in Good Repair (repeated violation)
    Observation: Ceiling of facility is not maintained in good repair. Also the floor & walls in the mop sink area not in good repair.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

    Observation: Floors under several pieces of equipment noted in need of cleaning.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

    Observation: Observed methods are not being used to control pests. Multiple flies seen throughout facility.

    Observation: Container of degreaser is not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items. Degreaser observed on shelf with food.
    Correction: Containers of hazardous products must be located in an area that is not above food, equipment, utensils, linens or single service items.

Recheck of small refrigerator on front line. Thermometer reading 41 degrees F. Temperature of foods were 45/46/50 degrees F. Foods were discarded. Unit will be checked.

No violation noted during this evaluation.
03/13/2014Routine
Visited facility due to fire in the french fry deep fryer. Check facility for reopening. All food thrown away including condiments. All paper products thrown away. Hot holding trays & condiment trays have been washed & sanitized. Hot holding units have been turned off & cleaned. Grease has been changed in chicken fryers. New fry fryer on order. Will be here in 4-5 weeks. Fries will be cooked in chicken fryers until new fryer arrives. Ok given to reopen.
No violation noted during this evaluation.

  • Plumbing System Maintained in Good Repair
    Observation: Plumbing connections under the 3 vat sink are leaking. Also leaking pipes in hallway outside employee break room, floor near utility sink and floor behind front counter.
    Correction: Plumbing systems and components shall be maintained in good repair. -- Per manager, plumber has been contacted and is looking for solution to problem.
