    Observation: A manufacturer's data plate was not found on the Auto-Chlor warewash machine.
    Correction: A warewash machine must be provide with an easily accessible and readable data plate.

    Correction: Remove this item from the facility and replace with an approved unit as needed. Equipment and utensils must be capable of maintaining their original characteristics in order to be easily cleaned and to prevent contamination of food.

NOTE: THE ESTABLISHMENT CONTINUES TO HAVE A FLY PROBLEMS. NUMEROUS COMPLAINTS RELATING TO THIS THE ISSUE HAVE BEEN RAISED WITH MANAGEMENT ON NUMEROUS OCCASIONS. TO DATE IT APPEARS NO ACTION HAS BEEN TAKEN TO ABATE OR RECTIFY THE ISSUE

    Observation: The person in charge could not provide a food temperature measuring device for the small front end refrigerstor.
    Correction: Obtain at least one food temperature measuring device so employees can properly monitor the temperature of food items in the establishment.

    Observation: The following equipment food-contact surfaces were observed soiled to sight and touch: ice scoop lying on ice in the ice maker.
    Correction: Clean and sanitize these surfaces for food contact.
  • Equipment, Utensils, Linens, and Single-Service and Single-Use Articles
    Observation: Single service items observed unprotected from contamination - styrofoam food containers observed not being inverted with fod contact surgace facing down
    Correction: Store single service items in its original protective packaging or inverted in an approved dispenser.
  • Critical: Backflow Prevention, Air Gap*
    Observation: An air gap between the water supply outlet and the flood rim level mop sink is not observed. If this direct connection exists between the drinking water system and a source of contaminated water during times of negative pressure, contaminated water may be drawn into and foul the entire system.
    Correction: Provide an air gap with a diameter that is at least twice the diameter of the water supply inlet and not less that 1 inch for proper backflow prevention by the air gap.

    Observation: The door gasket to the reach-in refrigerator by thre serve window is in poor repair.
    Correction: Repair or replace the door gasket in accordance with the manufacturer's specifications.

Discussed observations and inspection report. Overall the facility kitchen appeared clean. Use of food gloves, hair restraints, pH sanitizer test kit and date marking observed. Clean food contact surfaces. Refrigeration temperatures, cold and hot foods within required limits.
NOTE": Suggest to eventually replace gasket on faucet of 3 bay sink to prevent continous dripping
