Observation: Observed the left faucet of the 4 compartment sink has a leak.
Correction: Have leak repaired.

Observation: Observed the grout around the grill and the fryers is worn out and needs to be replaced. The front counter is broken in several places and has exposed wood.
Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities. Plans to remodel facility are set for January or February.

Observation: Observed raw formed burger patties made this morning in the walk-in between 43-46 degrees.
Correction: Don't fill up the bins so full and leave partially uncovered until the burgers cool back down to 41 degrees after making them.

Observation: Tomatoes in the prep unit were about 46 degrees.
Correction: Switched out remaining tomatoes with new ones from walk-in. Have maintenance look at prep unit and make sure it is working properly to hold food below 41 degrees.

Observation: Employee failed to wash hands after changing gloves from handling raw meat.
Correction: Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task.
