    Observation: A food service employee was working in the food service establishment without having a valid food service card.
    Correction: A food service employee shall have a valid food service card prior to working in the food service establishment. The person in charge shall not employ anyone to work in the food service establishment until they posses a valid food service card. A food service employee shall have a valid food service card while working in a food establishment.
  • Food Storage Containers; Identified with Common Name of Food (repeated violation)
    Observation: Unlabeled food containers.( Dunkin Donut Cup used for dispensing sugar with spoon).
    Correction: Label working containers with the common name of its contents. Food containers shall be labeled in a language that is understood by the regulatory authority and by the employee. Consumers may be allergic to certain foods or ingredients. The mistaken use of an ingredient when the consumer has requested that it not be used may result in severe medical consequences. Liquid foods and granular foods may resemble cleaning compounds. The mistaken use of food from an unlabeled container could result in chemical poisoning.
  • Temperature Measuring Devices (repeated violation)
    Observation: There was no temperature measuring device located in the Walkin cooler/Walkin Freezer, and lowboy service counter coolers.
    Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.
  • Equipment - Good Repair and Proper Adjustment
    Observation: Walkin freezer threshold was observed in a state of repair and condition that prevents necessary maintenance and easy cleaning.
    Correction: Repair the metal door threshold to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the equipment, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
  • Equipment - Good Repair and Proper Adjustment
    Observation: The door gasket of the milk,4 drawer, and Juice coolers are not fitting correctly in roller rack assembly:
    Correction: Repair or replace the cooler door gasket's in accordance with the manufacturer's specifications.
  • Critical: Handwashing - Using a Handwashing Lavatory*
    Observation: The handwashing facility located at the kitchen area is blocked, preventing access by employees for easy handwashing.
    Correction: Access to the handwashing facility identified above is to be available during all hours of operation. Remove the boxes and trash can which is preventing its use.
  • Outer Openings; Protected
    Observation: Outer opening of the food establishment is not protected against entry of insects and rodents.(Gap at back door/proper fit in disrepair).
    Correction: Protect the food establishment against the entry of insects and rodents by: 1. Filling or closing holes and other gaps along floors, walls, and ceilings

  • Equipment - Good Repair and Proper Adjustment
    Observation: Walkin freezer compressor/piping was observed in a state of repair and condition that prevents necessary maintenance and easy cleaning, along with theshold floor plating and door latch not in proper operation.(Buildup of ice on floors and shelving area, Evidence of leakage of water.Sugar storage container is in direpair(Cracked lid w/tape).
    Correction: Repair the unit/piping,door/floor theshold plating/door latch, to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the comprssor/piping, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.Proper operation IAW Specs, for all equipment,storage containers.
  • Physical Facilities in Good Repair
    Observation: Front service handsink is not maintained in good repair. (Non Operational)
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.Plumber called while on site.Employees washing hands in back prep area handsink,,till front service sink corrected for proper operation with hot and cold running water.

Note:Post CFM Certificate.Temperature logs/Charts not being kept on all refrigeration units 2x a day(open/close).All Food Handlers to be through an approved Food Handler Class.(Only Portsmouth is an approved on line course for Norfolk Health Department/or attend a Norfolk Public Health Department FHC through Norfolk Health Dept.No Basic Food Handler's Classes accepted,except Portsmouth Health Department's class.(online service only).

    Correction: Cold hold potentially hazardous food at 41°F or below to inhibit the growth of harmful bacteria.Food products voluntarily discarded.
  • Equipment - Good Repair and Proper Adjustment
    Observation: Prep Station 4 drawer unit was observed in a state of repair and condition that prevents necessary maintenance and easy cleaning.(Ice buildup/compressor area).Access lid left open at all times causing ice buildup).
    Correction: Repair/Fix/Hold/ temperature at ,41*F to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the cooler or hold proper temperature of unit/food, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
  • Hand Drying Provision
    Observation: No disposable towels were provided a the hand washing lavatory in the back and front area handsinks.
    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials.Corrected on site.
