    Correction: Provide an easily readable thermometer so employees can accurately read the thermometer.Corrected.

    Observation: There was no temperature measuring device located in the Salad prep loyboy cooler.
    Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.

    Observation: ES 2000 automatic dishwasher was observed in a state of repair and condition that prevents necessary maintenance and easy cleaning.(Suction hose/Squeese tube for sanitizer was worn).
    Correction: Repair the dishwasher to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. EcoLab called to correct issue,Issue was corrected on site.Sanitizer reading 50-60 ppm chlorine.

    Observation: Handsink in restroom was observed in a state of repair and condition that prevents necessary maintenance and easy cleaning.(Loose attachment of sink and caulking needs to be replaced and secured to sink and wall area.
    Correction: Repair the handsink to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the handsink,replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
  • Wall and Ceiling Coverings and Coatings
    Observation: Wall covering paint in pizza prep room(Behind prep table) is not attached so it is easily cleanable.(Paint chipped and peeling).
    Correction: Attach wall covering so it is easily cleanable or replace with protective covering.

    Observation: Pizza Prep area room ceiling leaking water and is not maintained in good repair
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.Landlord notified and work order has been issue.

Keddrell)).Discussed via phone conversation.

    Observation: Pizza oven pans were observed in a state of disrepair and damaged.(Rims of pans-cracked)
    Correction: Replace/Discard the pizza oven pans to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter.Replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.(Discarded on site,ones that are damaged).Maintain and Monitor condition of pans daily,for good condition.
  • Critical: Handwashing - Using a Handwashing Lavatory*
    Observation: The handwashing facility/sink located in the Prep Room was blocked, preventing access by employees for easy handwashing.
    Correction: Access to the handwashing facility identified above is to be available during all hours of operation. Remove the pots,pans,Fan,etc which is preventing its use.Corrected on site.

    Observation: (2) Deep fryer baskets(Catch wire is in disrepair) was observed in a state of repair and condition that prevents necessary maintenance and easy cleaning.
    Correction: Repair the Strainer baskets to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the the strainer baskets, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
  • Warewashing Machines, Manufacturers' Operating Instructions
    Observation: The mechanical warewashing machine was not operating in accordance with the manufacturer's specifications.(Zero sanitizer solution reading).Vaccum Breaker was also leaking.
    Correction: Operate the warewashing machine per affixed data plate specifications to ensure that equipment and utensils are properly cleaned and sanitized.Techician was called from Ecolab to correct problem with sanitizer feed lines/squeeze lines.Corrected on site with Ecolab.Proper reading recorded at 50 ppm chlorine.All new lines installed/sanitizer on machine.New vaccum breaker was installed on site, also.
  • Physical Facilities in Good Repair
    Observation: Ceiling/Roof in Dough room/Walkin cooler area is not maintained in good repair.(Roof leaking water to floor area).Landlord called to correct/fix this issue.
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.
