    Observation: A food employee failed to wash his or her hands before engaging in food preparation, after touching bare human body parts, after coughing, sneezing, eating, after handling soiled utensils or after engaging in any activity which may have contaminated his/her hands. Observed employee use the Rear Service door and then touch a clean knife prior to washing his hands. Observed.employee start to engage in food preparation after using the back service door prior to washing his hands.
    Correction: Instruct food employees to clean their hands and exposed portions of their arms immediately before engaging in food preparation including working with exposed food, clean equipment and utensils, and unwrapped single-service and single-use articles and during food preparation, as often as necessary to remove soil and contamination and to prevent cross contamination when changing task.

    Observation: The Keg Reach in Cooler Door was observed in a state of disrepair and damaged. The Keg Cooler Door was observed broken.
    Correction: Repair the door to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the door, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

    Observation: Surfaces of the Ice Machine Interior were observed soiled with accumulations of grime and debris.
    Correction: Clean the surface of Ice Machine Interior at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications.

    Observation: Containers of Kitchen Cleaners and Bleach were observed not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items. Observed chemicals stored on top of the ice machine and on the dishmachine.
    Correction: Containers of Bleach and other Cleaners must be located in an area that is not above food, equipment, utensils, linens or single service items.

    Correction: Minimize bare hand and arm contact with exposed food that is in a RTE form to reduce the potential for contamination.

    Observation: The Prepared Salsa in the Ice Bath was observed cold holding at improper temperatures.

    Observation: The prepared ready-to-eat (RTE) Refried Beans, Prepared Cilantro and Prepared Salsa in the walk in cooler were not properly dated for disposition.
    Correction: Mark the name and "consume by" date on the container of RTE foods at the time of preparation if the food is to be held for more than 24 hours. If the food is held at 41°F or below the food shall be served or sold within 7 calendar days.     Observation: Dispensing utensils improperly stored between uses. Observed dispensing labels and scoops stored on sodas in the walk in cooler.
    Correction: Store in-use utensils or dispensing utensils in one of the following manners: 1) In the food with their handles above the top of the food and the container. 2) In food that is not potentially hazardous with their handles above the top of the food within containers or equipment that can be closed. 3) On a clean portion of the food preparation table or cooking equipment only if the in-use utensil and the food-contact surface of the food preparation table or cooking equipment are cleaned and sanitized every 4 hours. 4) In running water of sufficient velocity to flush particulates to the drain. 5) In a clean, protected location if the utensils are used only with a food that is not potentially hazardous. 6) In a container of water if the water is maintained at a temperature of at least 135°F.

    Observation: Raw food of animal origin holding in a manner that may cause cross contamination of to ready-to-eat food (RTE). Observed Shell eggs above fruit and cut vegetables in the walk in cooler. Observed Raw Shrimp and pork above Tortilla's
    Correction: Separate raw foods during storage, preparation, holding, and display from raw RTE food including other raw food such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.

    Observation: Observed a container of Milk and a container of Half and Half Creamer in the bar reach in cooler cold holding at improper temperatures.
    Correction: Discard the food and ensure that potentially hazardous foods for cold holding are maintained at 41°F or below to inhibit the growth of harmful bacteria.

    Observation: Chlorine sanitizing solution used was not at an acceptable concentration. Observed a Chlorine concentration of less than or equal to 10ppm in the dishmachine.
    Correction: Immediately discontinue the use of the machine until it is operating in accordance with minimum requirements. Initiate manual wash, rinse, and sanitize method to properly clean and sanitize equipment and utensils. To minimize workload you can move to all single service utensils.
  • Handwashing - Using a Handwashing Lavatory - No Other Purpose (corrected on site)
    Observation: The handwash station at the Bar area is being used for purposes other than washing hands. Observed Hand sink being used to hold pitcher of bleach water.
    Correction: The handwash facility identified above is to be used for washing hands only

    Observation: Pesticides are not being applied by a certified applicator. Observed two Spray cans of Raid Pesticide stored in the kitchen facility under the three compartment sink. Person in charge stated they spray the pesticide in the kitchen facility.
    Correction: Restricted used pesticides are to be applied by an applicator certified as defined in 7 USC 136(e) Certified Applicator, of the Federal Insecticide, Rodenticide Act, or a person under the direct supervision of a certified applicator.
