    Observation: The rear hand sink is not sealed to adjoining walls.
    Correction: Seal the unit to adjoining equipment or walls since it is exposed to spillage or seepage.

    Observation: The sandwich prep unit was observed in a state of disrepair and damaged.
    Correction: Repair the equipment to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the equipment, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

    Observation: The following nonfood contact surfaces have accumulations of grime and debris: pizza oven, cabinet under the fryer, sprayer hose, and the pizza paddle
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.
  • Light Bulbs Protective Shielding
    Observation: Light bulb over the 3-compartment sink are not shielded, coated, or otherwise shatter-resistant.
    Correction: Shield or replace light bulb with a coated or shatter-resistant bulb.
  • Handwashing Cleanser - Availability
    Observation: Soap was not provided at the rear hand sink.
    Correction: Hand soap must be provided at all hand washing lavatories to encourage proper hand washing. Hand cleanser must always be present to aid in reducing microorganisms and particulate matter found on hands.

    Observation: No disposable towels were provided at both hand washing sinks. Paper towels were provided at the front hand sink.
    Correction: Hand drying devices such as individual disposable paper towels, a continuous towel system that supplies the user with a clean towel or heated air hand drying device must be provided at all hand washing lavatories to encourage proper hand washing and avoid employees to drying their hands on their clothing or other unclean materials

    Observation: The following physical structures are not maintained in good repair: light out in hood system and light out in the walk in freezer
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

    Observation: The following physical structures are noted in need of cleaning: floor behind the equipment on the prep line, water pipes near the grease trap, cob webs under the 3-compartment sink, and the air vent cover in the walk in cooler.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food. A regular cleaning schedule should be established and followed to maintain the facility in a clean and sanitary manner.

    Observation: REPEAT The person in charge failed to state the minimum cook temperatures and times for potentially hazardous food.
    Correction: Ensure the designated person in charge (PIC) is knowledgeable about foodborne disease prevention, Hazardous Analysis Critical Control Point principles and Regulation requirements. The PIC should be prepared to recognize conditions that may contribute to foodborne illness. The PIC must also recognize conditions that fail to comply with Regulation requirements and be authorized to take the appropriate preventive and corrective actions to ensure that the Regulations' objectives are fulfilled.
  • Critical: Package Integrity*
    Observation: Observed damaged can of pizza sauce on rear prep table. Food packages are not in good condition or the integrity of the package contents are not protected from adulteration or potential contaminants (ex: dented food cans)
    Correction: Discard or return to distributor any food packages that are not in good condition or the integrity of the package contents are not protected from adulteration or potential contaminants.
  • Critical: Food - Packaged and Unpackaged - Separation, Packaging, and Segregation* (corrected on site) (repeated violation)
    Observation: Observed raw hamburger stored between pre cooked sausage and sliced ham in the sandwich prep unit. Raw food of animal origin holding in a manner that may cause cross contamination of to ready-to-eat food (RTE). Items were rearranged.
    Correction: Separate raw foods during storage, preparation, holding, and display from raw RTE food including other raw food such as fish for sushi or molluscan shellfish, or other raw RTE food such as vegetables, and cooked RTE food.
  • Critical: Food - Potentially Hazardous Food - Hot Holding* (corrected on site)
    Observation: REPEAT Observed ham, egg, & cheese sandwich - 127 and chicken thigh - 127'F hot holding at improper temperatures. Foods were discarded.
    Correction: Rapidly reheat the food to 165°F and maintain at 135°F or above through the hot holding period.
  • Critical: Ready-to-Eat - Potentially Hazardous Food - Disposition*
    Observation: The commercially processed ready-to-eat (RTE) cole slaw mixed, pizza sauce, and sliced ham were not discarded by the "consume by" date. Items expired on 12/8/12. The cole slaw mixed and sliced ham were discarded.
    Correction: Discard the food at this time and ensure all commercially processed RTE food is served, sold or discarded by the ""consume by"" date after opening. Some harmful bacteria continue to grow even at refrigeration temperatures so limiting the amount of time in storage limits the amount of growth allowed for these bacteria.
  • Sponges, Use Limitation
    Observation: REPEAT Sponges are being used to wipe down food contact surface of the equipment.
    Correction: Discontinue use of sponges for this purpose as they are to be used only where they will not contaminate cleaned and sanitized, or in-use food contact surfaces.

If unable to repair the equipment, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.

    Observation: The cutting boards on the equipment storage rack in the rear are heavily scratched and scored. The food contact surface is no longer easily cleaned and sanitized due to condition.
    Correction: Resurface or discard and replace the cutting boards to provide a food contact surface that is smooth, easily cleaned and sanitized.

    Observation: The following equipment food-contact surfaces were observed soiled to sight and touch: fryer baskets, can opener, cone shifter on the equipment rack, inside of the stainless steel pans, and the colander.
    Correction: Clean and sanitize these surfaces for food contact.
  • Non-Food Contact Surfaces
    Observation: REPEAT The following nonfood contact surfaces have accumulations of grime and debris: caulking on the front hand sink, top of the pizza oven, interior of the pizza oven, can opener holder, and the white plastic utensil bin on the equipment rack.
    Correction: Clean the surface at a frequency necessary to preclude accumulation of soil residues that become encrusted or attract insects and rodents.

    Observation: Clean stainless steel pans were observed stored with the food-contact surface facing upward.
    Correction: Store equipment and utensils covered or inverted to prevent contamination while in storage.

    Observation: Less than 50 foot candles of light was noted in the prep area. Light measured 20 foot candles.
    Correction: Provide at least 50 foot candles at a surface where a food employee is working with food or working with utensils or equipment such as knives, slicers, grinders, or saws where employee safety is a factor

    Observation: The following physical structures are noted in need of cleaning: light shields throughout the entire facility, caulking on the 3-compartment sink, floor drain near the 3-compartment sink, floor under the fryer, air vent covers in the walk in cooler, air vent covers in the women's restroom, and the walls over the rear prep table.
    Correction: All floors, walls, and ceilings must be cleaned as often as necessary to keep them clean. Cleaning of the physical facilities is an important measure in ensuring the protection and sanitary preparation of food.
