The purpose of today's visit was to conduct a risk factor assessment. The following suggestions have been made as a means to incorporate Active Managerial Control:
1. Review Employee Health Policy with all long term employees regularly (i.e., monthly)

The purpose of today's visit was to conduct a routine assessment. Please continue to use safe food handling practices as means to promote public health. Recommended the use of equipment and food temperature logs as a way to both monitor cold holding practices and incorporate Active Managerial Control (AMC).
  • Nonfood-Contact Surfaces Clean at Frequency to Preclude Accumulation
    Observation: The nonfood-contact surfaces of the following equipment had accumulations of grime and debris: COMMERCIAL MICROWAVE.
    Correction: PLEASE CLEAN AND SANITIZE MICROWAVE REGULARLY TO AVOID ACCUMULATION OF DEBRIS. Nonfood-contact surfaces of equipment and utensils shall be cleaned at a frequency necessary to preclude accumulation of soil residues.

The purpose of this visit was to conduct a routine inspection.
  • Floor / Wall / Ceiling / Cleanability
    Observation: Floors, walls, ceilings and under prep tables were observed with food debris, dirt and dust.
    Correction: Clean all floors, walls, ceilings and under prep tables to prevent contamination to food and non food contact surfaces.
  • Physical Facilities Good Repair
    Observation: Observed that the gaskets on the 2dr prep cooler are torn and are not maintained in good repair.
    Correction: Replace the gaskets. Poor repair and maintenance compromises the functionality of the prep cooler.
12/03/2014Routine
Complaint states sewage backup at the facility. Environmental Health Specialist(EHS) spoke with Certified Food Manager(CFM) during time of investigation. CFM stated that the line was snacked to determine location of blockage, resulting in tearing up flooring. Blockage was located and repaired. Clean-out and floors were cleaned and sanitized. EHS did not observe a sewage backup during time of investigation, all repairs have been made and area sanitized. CFM stated that the kitchen has been closed since the problem began and it is closed today. Facility has hired professional cleaning company to thoroughly clean the establishment as an added precaution. After the professional cleaning, the kitchen will re-open.

    Observation: There is no test kit located in the facility for monitoring the concentration of the chemical sanitizing solutions.
    Correction: Obtain a BLEACH test kit.
  • Clean Ice Bins, Beverage Dispenser Nozzles, Etc. per Manufacturer Specs. (corrected on site)
    Observation: Surfaces of ice bins, beverage dispensing nozzles, and enclosed components of equipment are not being cleaned as required.--HOSHIZAKI ICE MACHINE INTERIOR-RIGHT SIDE-OBSERVED SOILED.
    Correction: Surfaces of utensils and equipment contacting food that is NOT potentially hazardous (time/temperature control for safety food) such as ice bins, beverage dispensing nozzles, and enclosed components of equipment such as ice makers, cooking oil storage tanks and distribution lines, beverage and syrup dispensing lines or tubes, coffee bean grinders, and water vending equipment at a frequency specified by the manufacturer or at a frequency necessary to preclude accumulation of soil or mold.
  • Microwave Oven Interior & Door Seals Cleaned Every 24 Hours (repeated violation)
    Observation: The (cavity, door seal) of the microwave oven is observed soiled.--AMANA MICROWAVE INTERIOR SOILED.
    Correction: The interiors of all microwave ovens including cavities, door seals, floors, walls, and ceilings shall be cleaned at least every 24 hours by using the manufacturer's recommended cleaning procedure.
  • Light Bulbs / Locations where Shielding is Required
    Observation: Light bulb(s) in the DRY GOODS STOARGE are not covered by a protective shielding.
    Correction: Provide light bulbs that are shielded, coated, or otherwise shatter-resistant in areas where there is exposed food, clean equipment, utensils, linens, or unwrapped single-service and single-use articles.
