    Correction: A food service employee shall have a valid food service card prior to working in the food service establishment. The person in charge shall not employ anyone to work in the food service establishment until they posses a valid food service card. A food service employee shall have a valid food service card while working in a food establishment.

    Observation: Unlabeled containers. ( Squeeze Bottles/Toxins)
    Correction: Label working containers with the common name of its contents. Containers shall be labeled in a language that is understood by the regulatory authority and by the employee. The mistaken use of these chemicasl/Toxin from an unlabeled container, could result in chemical poisoning.

    Observation: Wiping cloths improperly stored between use.(On front handsink area)
    Correction: Ensure wet wiping cloths are stored in a chemical sanitizer at the proper concentration between use.

    Observation: There was no temperature measuring device located in the lowboy refrigerators and walkin cooler.
    Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.Corrected.

    Correction: Clean/sanitize the ice chute shield unit to provide a clean opening of the container receiving the food/ice..

    Observation: Condiment lowboy refrigerator was observed in a state of repair and condition that prevents necessary maintenance and easy cleaning.(Leaking condensate water to bottom shelve area/pooling water on shelve.
    Correction: Repair the refrigerator to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the cooler, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
  • Critical: Equipment - Food Contact Surfaces and Utensils
    Observation: Surfaces of interiior shelve/bottom of the Condiments 1 dr lowboy refrigerator, was in contact with non-potentially hazardous food items that were observed soiled with accumulations of grime and debris/ condensate water leakage on bottom shelve area.
    Correction: Clean the surface of the bottom shelve arer at any time when contamination may have occurred, at least every 24 hours, before restocking the consumer self-service equipment/utensils, or per manufacturer specifications.Corrected by cleaning.
  • Critical: Toxics - Storage of Toxic Containers*
    Observation: Containers of Hazardous Products are not properly stored to prevent the contamination of food, equipment, utensils, linens or single service items.Squeeze bottles of chemicals stored/Hanging on splash Shield at front hand sink area.
    Correction: Containers of @HAZARDOUS PRODUCT@ must be located in an area that is not above food, equipment, utensils, linens or single service items.Corrrected.

    Observation: 1 dr lowboy cooler for chilli & Cheese, & Condiments was observed in a state of repair and condition that prevents necessary maintenance and easy cleaning.(Leaking water/Condensate, and rags used for soaking up water on bottom shleve area of interior area of unit.
    Correction: Repair/Replace the refrigeration uniit to restore a state of condition that allows for proper operation, accuracy, functioning, maintenance, and cleanability per Part IV, Article 1 and 2 of this chapter. If unable to repair the cooler, replace it with one that meets the specifications of Part IV, Article 1 and 2 of this chapter.
  • Mops - Drying Mops
    Observation: Mops not hung up to air dry.
    Correction: Wet use maintenance items such as mops must be hung in a position that allows them to air dry so they are not allowed to grow contamination which can then be spread through the establishment at the next use.

    Observation: There was no temperature measuring device located in the lowboy refrigerators.(Interior of units)..
    Correction: Provide a temperature measuring device in all hot or cold holding equipment used to store potentially hazardous foods so that employees can routinely monitor the ambient air temperature.
  • Critical: Backflow Prevention Device, When Required*
    Observation: Observed a hose attached to a faucet fixture/Hose at mop sink.. The hose extended below the flood rim level of the sink basin.
    Correction: Install an approved backflow prevention device or alter the length of the hose to provide the necessary separation between the water supply and the flood rim level of the sink basin. The minimum allowable separation distance must be at least 2x the diameter of the water supply inlet and at least 1 inch.
  • Physical Facilities in Good Repair
    Observation: Handsink next to 3 compartment sink in back office area, is not maintained in good repair. (Not secured to wall/Hanger bracket).
    Correction: Maintain physical facilities in good repair. Poor repair and maintenance compromises the functionality of the physical facilities.

    Observation: There is not a sufficient hot water supply (Temp, reading at 95*F) to the front hand sink and restroom area for proper sanitization within this food service establishment.
    Correction: Minmium temperature required to be 100*F. Mgt.Contacted Field Maintenance to come out to correct hot water supply issue by 7/12/2013.
