- Observed a container of lobster meat dated 12/31 and 6 individual containers of queso labeled 2/3. Per the manager the dates are from when the food is put in the freezer. We discussed labeling the food when it is pulled from the freezer. The items out of date were voluntarily thrown away by the manager.
- Observed a zip lock bag of meatballs for bloody mary's in the walk in cooler that did not have a label. There was a 2 gallon container of milk in the walk in cooler that was open and did not have a date. The manager labeled both items upon inspection. All potentially hazardous ready to eat foods should be labeled and used within 7 days.
- Observed rust accumulation on the lid of the exterior ice freezer and on the white chest freezer in the walk-in cooler. The pot rack above the prep table is not sealed. Repaint or repair the areas.
- Observed a chlorine sanitizer bucket behind the bar at approximately 10 ppm. I discussed proper sanitizer concentration with an employee. New sanitizer was provided at 100-200 ppm.
- Observed the potato peeler attached to the wall with food build up. Routinely wash, rinse, and sanitize.
- Observed a gap at the bottom of the back kitchen door. The gap should be sealed so that no daylight can be seen.
- Observed the whole wall with the hand sink in the kitchen to have debris build up. Clean and maintain routine cleaning.
- Observed an employees jacket stored on a prep table in the kitchen. Employee personal items should not be stored on food or food contact surfaces.
- Observed a can of pesticide stored with sanitizers under the 3 compartment sink. The pesticide was discarded during the inspection. Approved pesticides that are used in the facility should be stored physically separate from chemicals.

- Observed 4 ziploc bags of lunch meat labeled 9/25/15. Ready to eat potentially hazardous food must be used or discarded within 7 days of opening. These items were voluntarily discarded by the kitchen manager during this inspection.
- Observed thermometer at 40 F in ice bath. Thermometers should be routinely calibrated so that they read 32 F in ice bath. The thermometer was calibrated at the time of this inspection.
- Observed breading mixed stored on floor in exterior dry storage. Food items must be stored a minimum of 6 inches off floor.
- Observed kitchen staff without the use of hair restraints. Ensure effective hair restraints are worn.
- Observed rust accumulation on the lid of the exterior ice freezer and on the white chest freezer in the walk-in cooler. Replace or repaint. Observed crack in kitchen handsink. Replace.
- Observed debris accumulation on the following items/surfaces: Exterior ice freezer, 2 door silver freezer in kitchen, and soda gun holders in bar. Wash, rinse, and sanitize these items/surfaces and ensure routine cleaning.
- Observed utensils stored on boxes on the floor in the server area. Ensure clean utensils are stored a minimum of 6 inches off floor.
- Observed cracked and broken strainer that is used to carry clean utensils. Replace strainer.
- Observed single service items stored on the floor in the exterior dry storage. Single service items must be stored a minimum of 6 inches off floor.
- Observed single service items in re-use. Ensure single service items are not reused.
- Observed flooring in server area in disrepair. Repair.
- Observed debris accumulation on the floor in the walk in cooler and in the exterior dry storage. Clean and ensure routine cleaning.
- Observed debris accumulation on the ceiling in the server area. Clean and ensure routine cleaning.
- Facility has not met food handler requirements. Ensure all food handlers receive food handlers certification within 30 days of hire.

- Non food contact surfaces in disrepair: Shelves in reach in cooler are rusted, the seal to the same cooler is coming off, the chest freezer in the walk in cooler is rusted, non food contact surfaces shall be in good repair.
- Please defrost the white freezer in the kitchen.
- Found single service forks not dispensed with the handles presented. Also found clam shells with food contact surface stored up. Corrected.
- Found screen door in disrepair. Screen is being held on with duct tape. No sweep on screen door. Facility is going replace door in the next 2 weeks.
- Replace missing cove base behind toilets in women's room.
- Please clean the hood and the wall behind the grill area.
